1067351771 2003-10-27 Plug / Plug Back No Violations -On site for plugging of Rick Baker I/O well. Accompanied by Jeff Fry. Gary Watts contracted to plug the well. Gary hired Hoops Excavating to demolish the cinderblock well house and dig the brine pit. Cinderblock wellhouse torn down prior to arriving and was hauled to demo debris dump. Brine pit dug while on site. Moraine Materials brought in class A cement to plug the well in two separate trucks. 6 5/8 pipe left in the ground due to the overhead lines nearby. Hole diameter was 6 1/4 according to Gary. Solid plastic tubing used to fill the hole with cement. Hole depth was 789 feet according to Gary and was a Trenton Formation well. Entire hole filled. Cement weight ranged from 12.8 to 13.7 lbs/gal based on Garys mud scales. Top of well casing was located about 3 feet below the ground surface in pit. Gary had well identification plate made and was going to get prongs welded on to fit over the well. Brine in the brine pit to be pumped and hauled to an Indiana brine disposal well in the next day or so. Brine pit and the pit around the well to be filled in, graded, seeded, and mulched. Gary to send in copies of the cement tickets. Both Jeff and Rick took photos of the plugging. At the time that the site was left, cement was setting up and there was very little lose or drop of the plug in the well.
1067376346 2003-10-28 Plug / Plug Back No Violations -Preliminary Restoration Ispection. Met with Rick Baker, landowner, on site. Talked to Gary Watts, plugging contractor by phone later in the day. Since plugging finished on 10/27/03, brine in brine pit pumped out and sent to brine disposal well in Indiana. Brine pit filled back in as well as the pit where the well was located. Gary placed the ID plate on the well before filling well pit back in. The filled in brine pit and well pit has been graded, seeded, fertilized, and mulched. Gary indicated that he reseeded the area with rye grass. Photos taken of the preliminary restoration. Rick Baker was happy with the restoration work to date. Notified both that I would check the site in several weeks for the germination of the seed and would recommend approval of the final restoration if the seed is becoming established.

1068739999 2003-11-12 Final Restoration No Violations -Final Restoration Inspection: Plugged well area and brine disposal pit were covered, graded, seeded, fertilized, and mulched on 10/28/03. Since preliminary restoration inspection on 10/28/03, grass seed has germinated and begun to revegetate the two areas. Landowner Rick Baker was satisfied/happy with the restoration work when I met him on 10/28/03. Site was a landowner grant I/O well site. Photos taken of the site.
1071063423 2003-12-09 Final Restoration No Violations -Status check on the I/O well plugged on 10/27/03. Preliminary restoration inspection on 10/28/03. Final restoration inspection on 11/12/03 recommended approval of payment of the landowner grant. On site for follow up status check of the site to see how the vegetation is coming. Gary Watts backfilled, resoiled, seeded, fertilized, and mulched the plugged well area and the brine pit area on 10/27/03 and 10/28/03. Grass had begun to emerge on 11/12/03 inspection. Since last inspection, grass has become more established in the two areas. Photos taken of the activity.
