Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0311544701 2018-03-12 Plug / Plug Back No Violations Inspection was made to complete the orphan well report to refer the well into the orphan program. The landowner was contacted during inspection. The well report was forwarded to Jeff Fry.

1518623008 2018-02-13 Production Wells No Violations The surface owner contacted the inspector and stated that this well was struck by farm equipment last fall and he was concerned about leaking gas and getting the well cut off so it would not be an obstruction in the field. The well was reported dry and plugged on the well completion report dated 10/24/72. The well was filled solid with drill cuttings and left with a 7x2 swage and shut in per the completion. The well was below grade and was marked as final restoration status. The impact from the farm equipment ripped the valve off the 7x2 swage and opened up the well. Gas is detectable over the top of the swage. The well needs to be reopened and replugged.
