Well consist of seven inch casing extending approximately 6 inches above ground level and has steel plate with welded handle on top of casing. Operator of well is seriously ill and is not capable at the present time to operate well. No identification is posted at site.
1085670667 2004-05-27 Production Wells Violations Noted Inspection with Jeff Fry. Well is capable of production but does not appear to have been produced recently due to rust noted on flywheel and pulley. Valves were open and there was pressure on the well. Storage tank on site appeared to be empty. There had been oil spilled at the tank site as noted by oil around front of tank. The area was approximately 10x20 feet. There was some new grass growing thru the spill, but most of the area was otherwise coated in oil. No ID at well site

1091113333 2004-07-29 Production Wells Violations Noted Spoke to Mr. Harmeyer by phone. Area has not yet been cleaned up because of wheat harvest. Discussed bio-remediation for site. Mr. Harmeyer indicated that he looked at the site and thought the spill was a one time incident while pumping from tank. That appears to be accurate since the ground beneath the vegetation does not appear to be saturated with oil. Oily vegetation will be removed, ground will be disced, then fertilizer and straw will be incorporated into ground. Mr. Harmeyer said that ID signs were being made for all well sites.
1092759313 2004-08-17 Production Wells Violations Noted Oil spill has not yet been cleaned up. Spoke to Mr. Harmeyer by phone at last inspection on 7/29/04. He indicated at that time that the area would be cleaned up within the week. Will discuss situation with supervisor Jeff Fry.

Will contact operator. No ID sign post at well site yet.
1103134048 2004-12-15 Production Wells No Violations Soil around tank has been replaced. Oil line that was previously laying on top of tank from well head is now directed into the tank. Discussion with Mr. Harmeyer after last inspection indicated that he thinks pressure from well caused oil to spill from line. Noted that gauge on well showed needle spiked over 60 psi. There does not appear to have been any recent spillage. ID sign is not yet posted. Mr. Harmeyer said well was in process of being transferred at last inspection. ID must be posted as required in Notice Of Violation dated 5/27/04.
1112818312 2005-04-06 Production Wells Violations Noted Pumpjack has been removed from well . Plate has been placed over the well. Post for ID sign is in place but no ID sign posted. Spoke to Terry Rockiki today who said he removed all rods and tubing from well as well as bailing the well. Pumpjack and tank will be removed from site. He thought that land was changing owners. The well is still the responsibility of D&H Oil. Notice of Violation issued 5/27/04 requires ID sign to be posted. Sent Richard Harmeyer fill in the blank ID sign on 10/7/04. If sign is not posted by 4/29/05 the Notice of Violation will be considered in non compliance and further enforcement action may be taken. Well must be plugged or produced.
1118338662 2005-06-09 Production Wells No Violations ID sign is posted at site. Pumpjack has been removed from well along with rods and tubing. Well will be produced with portable equipment. Plate on top of well casing for protection.. Storage tank is no longer connected to well and will be removed from site. NOV will be terminated.
