0283787529 2013-10-23 Administrative Inspection No Violations Operator submitted analytical results for soil that was stockpiled and bio-remediated. The operator requested approval for spreading the bio-remediated soils and approval was granted by Bob Warstal.
0297916927 2013-09-30 Production Wells No Violations Inspect well, location, and road ditch. The oil and sheen in the road ditch has been successfully removed from the water. The operator was advised that cleanup activities in road ditch are completed. The soil impacted in on a plastic liner and is covered with a plastic liner for bio-remediation. The pile is about 32 feet by 18 feet and 4 feet high. The division has requested the operator test the bio- remediated soils be tested for Total Petroleum Hydrocarbon (TPH) and Benzene, Toluene, Ethyl benzene, and Xylen (BTEX) prior to spreading on the farm field.
0322238216 2012-07-18 Drill / Deepen / Reopen No Violations At the time of my inspection the drilling crew was making preperations to continue drilling. Surface casing was cemented on 7/14/12 at 544.
0560685219 2013-08-14 Production Wells No Violations Inspected well, location, and road ditch. Inspected the cleanup activities in road ditch caused by uncontrolled release of oil from well head. Contacted operator to report that the oil absorbent booms were oil soaked, he stated the booms had been replaced the previous week. Will continue to monitor until all oil and sheen had been removed.

0905800853 2013-07-23 Production Wells No Violations Oil was observed in road ditch to the north of the location along County Road 174 and North of County Road 119. The oil has coated the grasses for about 1100 feet. This is a result of the blowout on 7/17/13 and local heavy rains of upwards of 7 inches after the oil spill release. Boom have been placed at locations every 250 feet to collect the oil. The drill rig is tripping in the hole to extend the measured depth of the borehole.
1002282973 2013-09-04 Production Wells No Violations Inspect well, location, and road ditch. Residual oil continues to be present in the road ditch. Contacted operator recommended changing the oil absorbent booms.

1375621881 2013-07-18 Production Wells No Violations M&M Royalty was drilling the lateral on air on July 17, 2013 at the Weickert 4-H (34143203220000) though the Trenton limestone in Sandusky County, Rice Township north of Freemont, Ohio. The measured depth was about 2650 feet. At approximately 9:30 pm the neighbors began to notice a petroleum odor near the C A Weickert 3-H (34143203210000). At approximately 10:00 pm the rig crew began to notice a petroleum odor and was checking the pits. The crew then observed a blowout at the Weichkert 3-H location. The M&M Royalty personnel, Mike Weinsz and Mat Egnotvich and several rig hands immediately arrived at the 3-H location to assess the situation and shut in the well. Emergency responders were contacted by others and were on site within 5 minutes of M&M Royalty. The responders included the local volunteer fire department, Emergency management, Sandusky County Sheriff, and the local Hazmat team. Mat (last name unknown) with the local fire department announced that he was in control and would notify OEPA and DNR. Inspector Benko received a phone message at 10:18 pm from the ODNR Communication center. Inspector Benko Contacted the Com Center at 1:56 AM and was told to disregard. Dave Schilt of the Ohio EPA contacted Inspector Benko at 7:00 am on July 18, 2013. M&M Royalty was then contacted and details were ascertained. In preparation to extend the lateral of the 3-H the pump jack was moved off the well and the rods and tubing were pulled out of the well. One joint of tubing was left in the well head and the valve for the 4.5 inch tubing remained open and the production line was not reconnected. A theory presented is there was communication between the drill rig and the existing 3-H lateral and caused the well to pressure up and blow out. The other theory is the well pressured up and blew out because it was shut in. Reportedly, there is not enough gas on the well to run the pump jack. Remediation included blowing the well down and flowing the oil into the production
