0349991515 2008-05-07 Plug / Plug Back No Violations On site to witness the bottom plug. Ran tubing to 1100 foot and pump 125sk of Type-1 cement. Shutdown for the day.

Will return to the site in the morning and measure in to the cement and top off.

1209580808 2008-04-30 Orphan No Violations Operator was pulling tubing from well when inspector arrived. Oil and gas began discharging from the well. Backhoe and vac truck contained oil to general area where previous oil was noted during review of well for I&O program. Bob indicated that because of wellhead style, that well could not be controlled when pulling tubing. Also the gas from the well would not allow welding on the casing. After well stopped flowing, well was shut in and vented to vac truck.. There was a pencil sized hole in the casing that will be patched. Salt water will be used to try to force oil and gas back into well. Spoke to Jeff Fry and Sam Faust for advice on situation. Operator was spreading peat moss and fertilizer on ground to absorb oil for remediation at end of inspection.
1210280987 2008-05-07 Plug / Plug Back No Violations Inspector delayed arriving at site. Operator pumped cement by reverse grouting to complete plug from depth 170 feet to surface. Discussed remediation of soil on site due to oil spilled during removal of tubing. There was existing oil on ground that discharged from the well prior to work by operator. Fertilizer and peat moss have been spread on ground. Area will be roto tilled and additional peat and fertilizer added as needed. This will be repeated several times.
1218658286 2008-08-12 Orphan No Violations Operator tilled peat moss and lime into area where oil spilled during plugging of well. Some other dirt also dumped on site. No vegetation on area yet but it has been very dry in past 3 weeks. No wet spots of oil noted. Will check area again to determine if need additional lime, peat moss or tilling before seeding this fall.
1226344834 2008-11-10 Orphan No Violations It does not appear as though any additional work has been done to restore well site. There is no vegetation on the site. Spoke to Bob Harpster by phone. Need to seed and establish vegetation or if landowner wants to turn area into agriculture as is surrounding area, a written request signed by landowner should be submitted.
1248705800 2009-07-15 Orphan No Violations Sparse vegetation on the site. Suggest adding lime, roto tilling lime and existing peat moss into ground and seeding.
1391205742 2009-12-02 Orphan No Violations Checked site for final restoration. Some of the area disturbed during plugging has been disc for farming and the remaing area has volunteer vegetation. No sign of oil contamination on ground. Final restoration is acceptable.
