0592419548 2012-03-31 Orphan No Violations At the time of my inspection, Black Swamp Oilfield Services was on site to start drilling on the wood plug in the 5 5/8 casing . Wood plug was pushed to 830 with a 5 5/8 cable tool bit. Bit started to stick and samples from bit revealed shale that had swelled due to the introduction of fresh water into the borehole. Well was shut in till decision is made on how to proceed.
0622846123 2012-04-02 Plug / Plug Back No Violations At the time of my inspection, Black Swamp Oilfield Services was on site to run 2 tubing to determine depth and flush oil from well to determine if shale and trenton formations have swelled shut. Oil flushed three times over a 6 hour period and less oil was removed each time with the last flush producing less than 10 gallons. Sandusky Co. inspector Tom Benko was on site to look at samples and watch the last flush. The decision was made to plug well from current depth of 830 to surface with cement. 1043603470 2012-04-03 Orphan No Violations At the time of my inspection, Black Swamp Oilfield Services was on site to tag cement from bottom plug set on 4/2/12. The top of cement was tagged at 305 and the well was again flushed with less than a gallon of oil in the hole. The cement was ordered to finish plug and was then cancelled due to thunderstorms in the area. Tubing was left in the hole and shut in.
1290511729 2012-03-30 Orphan No Violations At the time of my inspection, Black Swamp Oilfield Services was on site to start the plugging process. A new section of 5 5/8 casing was welded on to replace damaged surface casing and then reduced down to 2 with a valve and left to vent into the pit. Crews from the Woodville Township Fire Department on site during the welding process. Pit was constructed and the rest of the equipment put in place. Well was shut in till next morning.
1333640070 2012-03-29 Orphan No Violations At the time of my inspection the oil level in the 12 riser was rising so preperations were made, in coordination with USEPA (Jeff Lippert), to put another riser on and have a vac truck brought on site to remove oil from riser. Cleanup on impacted ditch had begun under supervision of USEPA.
1334056768 2012-04-06 Plug / Plug Back No Violations At the time of my inspection the rest of the well from 305 to surface with 42 sacks. Good cement returns to surface. See report for details.
