0506521955 2016-02-08 Production Wells No Violations At the time of this inspection fluid level inside the riser was one inch from overflowing. This represents a three inch rise from the inspection on 2-3-16. The fluid inside the riser appears to be mostly oil with a small amount of bubbling. There was no gas detected using the Sperian PhD 6 gas detector but the windy conditions were not ideal for taking a reading. The well will continue to be monitored for any further leaking and to check the integrity of the riser.
0625967314 2015-07-17 Production Wells No Violations At the time of the inspection I was on location with NE Regional Supervisor Tom Hill to spread some Peat-Sorb on the area and place riser. The well was not leaking at the time of the inspection and the area of impact has remained static. Two bags of Peat-Sorb were spread on location in the morning and an additional bag was added when the riser was placed in the afternoon. The riser was set using granular bentonite to create a seal to keep any leaking fluid inside the riser. This well will be referred for possible acceptance to the Orphan Well Program and will be monitored accordingly.

When we arrived on location is started raining so any cleanup work was postponed until the weather allows. The well was not leaking at the time of the inspection and the area of impact has remained static.

1089513580 2015-08-11 Production Wells No Violations At the time of the inspection the riser set on 7-17-2015 was still in place and the bentonite used to seal the space between the well and riser was expanding. There was evidence of some leaking from the well that stayed inside the riser. There were also some spots in the impacted area that had started pooling oil again. These pools were the result of some recent rains and warm weather releasing trapped oil from the impacted area. This well will be referred for possible acceptance to the Orphan Well Program and will be monitored accordingly.

The fluid level inside the riser has risen to the top and a small amount of oil has leaked down the outside of the riser. This well is under review for possible referral to the Orphan Well Program.

1945635823 2015-07-13 Production Wells No Violations This inspection was done at the request of the landowner, Larry Leow, when the well leaked oil into his woods during heavy rains in late June and early July. At the time of the inspection the well was not leaking but it is evident that the well has been leaking for many years as there is a 2,000 square feet area around the well that is visibly impacted with crude oil. In some areas the oil saturation reaches a depth of 6 below grade. This area has very shallow till and has bedrock at the surface in some places. The well is equipped with wooden conductor casing around 6 diameter surfaces casing. The well also has a wellhead with one open 2 fitting and one plugged 2 fitting. The open 2 fitting appears to be where the oil has escaped fro the well. An unknown amount of 2 diameter tubing on a ring was placed in the well and this tubing can be lifted by hand. The well appears to be a Historic Well that has not been produced for some time and the 6 diameter surface casing has failed allowing fresh water into the well. The decision was made to leave the well as is until the landowner can provide a riser pipe to place over the well and contain any further leakage from the well. API # 3414360386 was assigned and the well was added to our database on 8-3-2015.

2031780310 2016-02-03 Production Wells No Violations At the time of the inspection the corrugated riser was still in place and the bentonite, which had recently been applied to the outside base of the riser, has swelled and sealed off any leakage. The top of this riser is set at 4 above ground level. An eighth inch of rain was measured by the landowner overnight but no leaking was observed outside of the riser. The fluid level inside the riser was four inches from overflowing and this fluid appears to be mostly oil with a small amount of gas bubbling. There was no gas detected using the Sperian PhD 6 gas detector but the windy conditions were not ideal for taking a reading. The well will continue to be monitored for any further leaking and to check the integrity of the riser.
