0473738792 2012-11-26 Production Wells No Violations Vickie McKinney, ODNR O&G, Columbus, requested that this well should be inspected based on a telephone conversation she had with the landowner, Beulah Sherman (Mailing address: 3622 Gallia Street, Portsmouth, Ohio 45662, Scioto County; telephone: 740-456-6072). The owner stated that she has a gas well located on another property she owns on 1956 Blue Run Road, Lucasville, Jefferson Township. Mrs. Sherman was concerned that this well was still leaking gas even after the well was shut-in. Jon Scott and I went to the property to check on this well. The Sherman Residence was located behind another residence which was immediately adjacent to Blue Run Road (1974 Blue Run Road). We drove up the access road to the vacant house. Stopped the truck and walked to the house with Jon Scott continually monitoring for gas emissions using the Honeywell PhD6 gas meter. No gas emissions were detected. Found the well about 26 feet from the northeast corner of the back side of the house. It was housed in a low profile enclosure made of concrete block about 2.5-feet high and 2 by 4-feet, width and length respectively. The enclosure was covered with a sheet of plywood and sheet of fiberglass. Still monitoring for gas emissions we carefully pealed back the sheets to uncover the enclosure. Then we carefully pulled back fiberglass insulation to uncover the gas well. The description of the wellhead is as follows: a two-inch steel production tubing rose out of the ground. On top of this pipe is a two inch T-fitting on which rising vertically is a 1/2-inch pipe terminating in a 1/2-inch shut-in valve. Attached to the other available opening on this T- fitting is a 2-inch steel pipe progressing horizontally to a 2-inch master shut-in valve. From this valve a one-inch pipe continues horizontally to a 1-inch 90 degree elbow. Continuing on from this elbow there is another short 1-inch pipe to another t-fitting. 0764080649 2014-11-05 Exempt Well Pre Transfer No Violations I received a request from central office Kathryn Smith. Wanda Howard had applied for domestic status of this well. I met with Wanda Howard. She and her husband purchased land were the well is located. I informed Wanda Howard the well could not be placed into domestic status because the well was not being used for domestic use.
1956861337 2013-07-02 Production Wells No Violations The owner called me to report a suspected gas leak near her property. One of her neighbors was walking into the woods using the access road to Mrs. Shermans house. She smelled gas when she was near the barn between Blue Run Road and Mrs. Shermans house and reported it to Mrs. Sherman. I went to the location and monitored for gas from Blue Run Road to Mrs. Shermans house. The PhD6 gas meter detected no gas but as I was returning to Blue Run Road I detected the smell of gas near the barn. Still no response from my gas meter. As I got closer to the barn I noticed a propane gas cylinder on its side with a slow flow of gas coming out of the valve on top of the tank. I contacted the land owner and informed him that he had a leaking propane gas cylinder. He promised to shut it off. I contacted Mrs. Sherman about the situation and told her to contact me if her neighbor still continues to smell gas on future walks in that area.
