1189299179 2014-03-18 Production Wells No Violations I was on location to confirm wellhead coordinates and resolve discrepancies between the GIS and RBDMS databases. A plot of GPS coordinates versus the databases found that the RBDMS point plotted 1270 SW of the actual near the location of the Miller #1 (PN#21734) and GIS plotted 90 SE of the actual location. The well was equipped with an electric pumping unit and I could not find an ID sign at the wellhead. The tank battery is shared with Miller #1 and contains 2-100 barrel tanks, a 50 barrel poly, and a vertical separator with a domestic hook up. The battery is diked and there is an empty 100 barrel tank sitting outside the battery that is not in use. The current ID is incorrect and indicates the Kiko #s 1 & 2. I will inform the Stark County inspector of my findings.
1395150286 2014-03-18 Production Wells No Violations I was on location to update the GPS coordinates for this well and perform a status check. During my inspection I found a tank battery with the proper I.D. signage and contained two 210 bbl production tanks, 100 bbl poly brine tank and a vertical separator all contained in a SPCC dike. The pump jack well is electric driven and located in a field behind the tank battery. There are no violations at this time.
2090864409 2014-08-26 Production Wells No Violations I was on location to perform an inspection on this well in regards to complaint #4759. The lease road is in good condition. The tank battery contains two 100bbl production tanks, a vertical separator with a house gas connection and a poly brine tank. The tank battery has proper I.D. signage. There is a puddle of spilled oil being absorbed with absorbent pads, I called Willy Mack (330-465-3872) and told him to have them cleaned up and removed. The pumpjack well has 4.5 and tubing annulus pressure gauges, but are showing 0 psi. The 8.625 casing was buried when the well was drilled and put into production and is not currently accessible. I will follow up on the clean up in one week of the tank battery.
