| 1409154078 | 2014-08-27 | Production Wells | Violations Noted | I was contacted by Bruce Levengood (330-340-9645) and informed that a flow line leak had occurred the night before (8-26-2014) at this well. I met the contractor, Jeff Finley, on location to inspect the well and the spill. The well is a pump jack well with a house gas connection. The tank battery has I.D. signage and contains a dike and a 210bbl production tank. The flow line leak occurred in a low spot and released approximately 5bbls of crude oil. The spill covers two areas, one approximately 30x 20 and a second area approximately 80 x 5. At this time they have been covered with peat moss and is being dug up with shovels. The area is not accessible with a back hoe or excavator, therefore four wheelers are being used to bring in shovels and buckets to haul out contaminated soil and free crude oil. The oil soaked vegetation, free crude oil and contaminated soil must be cleaned up and removed from the location and properly disposed of. Adsorbent socks have been installed to prevent any flow of oil outside of the spill area and will contain the oil if a rain occurs. I will follow up on the location to ensure cleanup has occurred before the compliance notice will be closed. |

Four wheelers and small excavators are currently working to remove any free oil and contaminated soil from the location and dispose of it properly. Where cleanup has occurred fresh peat moss has been spread to absorb any residual oil left over. They expect cleanup to continue into tomorrow and maybe Saturday depending on how much contaminated soil they encounter. I will follow up following the weekend to ensure cleanup is complete and total. |

At this time the compliance notice has been closed and there are no violations. |
