0213151025 2015-08-13 Plug / Plug Back No Violations I was on location to tag the cement top from the previous days plugging. The cement top has been tagged at 94 below surface. A grout truck is going to be brought onto location to use a siphon string and top off the well with 9 sack grout. The well is dead and is not leaking at this time.
0566812116 2014-10-30 Production Wells Violations Noted I was on location to perform an exempt pre-transfer inspection for this well. Currently the well has a 4.5 gate valve, and a swage to a 2 diameter bull plug with a non-functioning pressure gauge. The well has no production control equipment and is not capable of production. There is no domestic supply equipment present. The 8.625 diameter casing is buried and not accessible. There are no visual integrity issues with the wellhead. The tank battery contains a 100bbl production tank and a vertical separator. The tank battery is heavily overgrown, and does not contain a legible I.D. sign. This well does not meet the definitional requirements for an Exempt Domestic Well, per 1509.01(X). This well is not capable of production and has not shown any production since 1984. This well shall be plugged per Ohio Revised Code 1509.12(B) within 180 days.
1196961823 2015-08-11 Plug / Plug Back No Violations I was on location to witness the plugging of this well by Bishop Well Services and Gonzoil. The rig ran 4250 of 2.375 diameter tubing into the wellbore and Petroset established circulation. A 30sk Class A 2% gel cement plug was pumped and displaced with 15bbls of fresh water. The well was shut in and plugging will continue on 8/12/2015.
1439388638 2015-08-12 Plug / Plug Back No Violations I was on location to witness the plugging of this well by Bishop Well Services and Gonzoil. The Bishop service rig ripped the 4.5 diameter casing at 3170 and pulled it up to 2950. Petroset pumped a 75sk Class A 2% gel 300 Big Lime plug through the 4.5 diameter casing and displaced it with 42bbls of brine. The casing was pulled to 998 and a 50sk 200 Class A 2% gel cement plug was pumped and displaced with 8bbls of fresh water. The casing was pulled to 200 and 75sk 200 Class A 2% gel cement plug was pumped and circulated to surface. The casing was pulled from the well and cement topped off. The cement top will be checked tomorrow 8/13/2015 to determine if a grouting cement job is needed. There are no violations at this time.
1439830625 2015-08-17 Plug / Plug Back No Violations I was on location to witness the top grouting of this well. The cement top was at 94 and 2 cubic yards of 9 sack grout were used to top off the hole. The grout came to surface and did not fall. The well casing will be cut down below grade and a metal plate with the API # affixed to the top. This well has been successfully plugged and the compliance notice has been closed.
1939223584 2015-10-19 Final Restoration No Violations I was on location to check the status of final restoration. The location has been reclaimed and vegetation has been established. All of the production equipment has been removed and there are no signs of the well leaking. A plugging report is on file for this well. At this time final restoration has been passed.
