1158858343 2014-10-30 Production Wells Violations Noted I was on location to perform an exempt pre-transfer inspection for this well. Currently the well has a 4.5 gate valve, and a swage to a 2 diameter bull plug with a non-functioning pressure gauge. This well has no production control equipment and is not capable of production. There is no domestic supply equipment present. The 8.625 diameter casing is buried and not accessible. There are no visual integrity issues with the wellhead. The tank battery contains a 210bbl production tank, a vertical separator and a completely full brine vault that needs to be emptied. The tank battery is heavily overgrown, and does not contain a legible I.D. sign. This well does not meet the definitional requirements for an Exempt Domestic Well, per 1509.01(X). This well is not capable of production and has not shown any production since 1996. This well shall be plugged per Ohio Revised Code 1509.12(B) within 180 days.
1405359713 2015-10-19 Final Restoration No Violations I was on location to check the status of final restoration. The location has been reclaimed and vegetation has been established. All of the production equipment has been removed and there are no signs of the well leaking. A landowner waiver has been received to waive the removal of the in-ground brine storage tank which has been cleaned out and crushed. Also waived is the removal of the 2 diameter flow line which will be left in place. A plugging report is on file for this well. At this time final restoration has been passed.
1439573192 2015-08-14 Plug / Plug Back No Violations I was on location to witness the setting of the Big Lime Plug for this well. The Bishop Well Service rig ran 2822 of 2.375 diameter tubing into the well. Petroset circulated the wellbore and pumped a 75sk 300 Class A 2% gel plug. The plug was displaced with 10bbls of fresh water. The rig will pull tubing to the bottom of the surface plug point and shut the well in for the weekend. Plugging will continue on 8/17/2015.
1440510761 2015-08-17 Plug / Plug Back No Violations I was on location to witness the plugging of this well by Gonzoil and Bishop Well Services. The rig ran 836 of 2.375 diameter tubing into the well. Petroset circulated the wellbore and pumped a 50sk 200 Class A 2% gel plug to cover the bottom of the surface casing. The plug was displaced with 2.5bbls of fresh water. The tubing was pulled to 200 and a 60sk Class A 2% gel cement plug was circulated to surface. The tubing was pulled from the well and cement did not fall. The casing will be cut below grade and a plate with the API# will be fixed on the well. This well has been successfully plugged and at this time the compliance notice for this well has been closed.
1829844892 2015-08-13 Plug / Plug Back No Violations I was on location to witness the setting of the bottom plug for this well. The Bishop Well Service rig ran 4093 of 2.375 diameter tubing into the well. Petroset circulated the wellbore and pumped a 30sk Class A 2% gel bottom plug. The plug was displaced with 15bbls of fresh water. The rig will rip casing and pull it from the well due to a hole in the 4.5 diameter casing.
