0237875961 2016-11-14 Production Wells No Violations I conducted a status check inspection on this well prior to the start of plugging operations. The well is equipped with a pumpjack with an electric motor. The equipment with in the tank battery has been removed. The tank battery is shared with well # 151-24094 (Garren #3) which will also be plugged.
0764016124 2015-03-06 Production Wells No Violations I was on location to perform an inspection of this production well. The lease road is stable and leads up to an electrically driven pumpjack in good condition. The tank battery is contained within a dike and has proper I.D. signage. The tank battery contains two (2) 210bbl production tanks, vertical separator and concrete brine vault. The tank battery is shared with well #24094. This well did not produce in 2013, but did commercially produce in 2012.
1298569437 2017-10-30 Final Restoration No Violations I did a final restoration inspection for this well. There is no visual sign of the well or tank battery. The final restoration cannot be passed at this time. There is a power pole with a breaker box still attached near the wells location which needs removed or a landowner waiver.. A landowner waiver is also needed for the flowlines to remain. An email was sent to Duane Clark (Petrox) on 10-30-2017.
1412419081 2017-10-30 Plug / Plug Back No Violations I did a compliance notice follow-up inspection on this well. According to the record no compliance notice was ever issued for this well. With that, the well was plugged on 09-21-2016. The compliance notice is passed
1460038126 2016-03-29 Production Wells No Violations I conducted a compliance notice follow-up inspection of this well. The wellhead has a pumpjack on it and the valves are closed. The two, 210 BBL production tanks have been removed from the tank battery, along with the concrete brine vault. The oil-stained soil around the base of the production tanks has been cleaned up and removed. Proof of proper disposal must be submitted to Inspector Mullins at the address listed below. There is also a domestic line with a meter, drip bottle and chart boxes which has been left onsite inside the dike. No new signage has been added to the remaining tank battery equipment or wells. The tank battery is shared with well #24094. The lease access road is stable and driveable. A Compliance Notice was issued to the operator on 3/11/15 requiring the well to be plugged within 180 days (9/11/15), as the well was not capable of production, and only zero production had been reported since 2013. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days, per OAC 1501:9-11-05(B). The well owner/operator will be notified if the plugging schedule is approved or denied. In the interim, the owner must correct the identification sign.
