The previous final restoration inspection done by Inspector Fred Romeo on 12-16-2014 was not passed since the area had not been seeded. My inspection found the site is well vegetated and there was no visual evidence of the well or production equipment. The final restoration is passed
0302493465 2012-06-26 Production Wells No Violations FOLLOW UP TO NOV # -1326818493 FOLLOW UP TO NOV # 1807093521 I received a call from Ed Zwick (330-413-4593) in regards to well on his property that was leaking gas. I met with Mr. Zwick to investigate the well. I found that the well head was equipped for a pumping unit; however, the pumping unit had been removed. The polish rod was still protruding from the top of the well head. There was no identification posted and the area around the well head was very grown up with vegetation. I did detect a slight gas odor but I did not hear gas hissing from the well, and there were no fluids leaking around the well head. This well shared a tank battery with permit #3339. The tank battery consisted of one 100 barrel stock and an earthen dike that was overgrown with vegetation. There was no brine storage tank, no separator or chart present. The tank was rusted and no identification was posted for permit #3340. There is no reported production for this well since 1993. I called Rusty Morrison (330-397-2094) and Pat Ervin (330-654-4343) of Atlas Energy/Viking Resources to report my findings and inform them that this well needs to be plugged.
0540569899 2012-09-12 Production Wells Violations Noted I received an email from Chris Grimm on August 28, 2012 regarding a phone call he received on June 6, 2012 from Edward Zwick. Mr. Zwick is concerned with the Oberlin Unit 2 (34-151-2-3340-00-00) leaking gas. Chris Grimm inspected the well and issued a compliance order on June 27, 2012. He found that the wellhead was equipped for a pumping unit; however, the pumping unit had been removed. The polish rod was still protruding from the top of the wellhead. There was no identification posted and the area around the wellhead was overgrown with vegetation. He did detect a slight gas odor but did not hear gas hissing from the well, and there were no fluids leaking around the wellhead. This well shared a tank battery with permit #3339. The tank battery consisted of one 100 barrel stock and an earthen dike that was overgrown with vegetation. There was no brine storage tank, no separator or chart present. The tank was rusted and no identification was posted for permit #3340. There is no reported production for this well since 1993.

I spoke to Mr. Zwick regarding the previous inspection and inspected the well on September 12, 2012. I found the same conditions as reported by Chris Grimm. There was a slight gas odor emitting from the well but no fluids on the ground. I spoke to Pat Ervin at Atlas Energy (330-654-4343) regarding the condition of this well and he requested a letter be mailed documenting the inspection. This well has no reported production since 1993 and must be plugged or produced.

1039034457 2014-12-16 Final Restoration No Violations I was on location to check the status of final restoration for this well. The well has been plugged and the production equipment removed from location. The location has been graded and cleaned but not seeded at this time. The compliance notice issued on 6/27/2012 requiring the well to be plugged has now been closed. I will follow up and pass final restoration when it has been completed and the location seeded.
1178124106 2007-04-03 Production Wells No Violations I received a call from Edward Zwick (330-280-7571) regarding the status of this well. Terry Whitehair (Atlas America) said that the well is idle and it will be plugged. Ed Zwick wants to purchase the well for domestic gas, but he does not own the mineral rights only the surface rights. If Ed Zwick acquires the mineral rights, Atlas America will sell him the well. If Ed cannot acquire the mineral rights, Atlas America will plug the well.

My inspection found that the well head was equipped for a pumping unit; however, the pumping unit had been removed. The polish rod was still protruding from the top of the well head. There was no identification posted and the area around the well head was very grown up with vegetation. I did not detect a gas odor and I did not hear gas hissing from the well. There were no fluids leaking around the well head. There is no reported production for this well since 1993. The lease road to the wellhead has been fenced off and is not accessible by vehicle. There has been a permit to plug this well issued on 12/19/2012 and expires 12/19/2014. I contacted Atlas Energy and they stated that the AFE for this well is currently being approved at their Texas office and that Nicks Well Plugging will be on location when available in the coming months. This well must be plugged.
