0147408314 2007-11-19 Annular Disposal No Violations There is no Annular Disposal currently in use. Remove this well from the A.D. list. There were no violations at the wellhead or at the tank battery. This well was previously inspected on 3-18-2003 for A.D. and no A.D. was in use at that time.
1203511563 2015-12-23 Production Wells No Violations I was on location to follow up to complaint #5172. The well has had the downhole pump replaced and new flowlines installed. The well has been put back into production. At this time this well is producing and the complaint can be closed.
1214247665 2008-06-23 Production Wells Violations Noted I received a call from Tim McConaha (Enervest) regarding a flow line leak at the well site. Troy Wells (Enervest) has already started the clean up.
1214337948 2008-06-24 Production Wells No Violations The crude oil clean up in the swamp area is continuing. The swamp area is contained and is not discharging. Tim McConaha estimated 6-7 bbls were released and is on the surface of the water. Enervest workers are using a row boat and a leaf blower to push the crude oil toward the booms and absorbent pads. The release was reported to ODNR, OEPA, and Stark Co. LEPC yesterday.

1446134729 2015-10-29 Production Wells No Violations I was on location to perform an inspection on this well regarding complaint #5172. The complainant stated that there is a well on his property that has not had the pumpjack run in three (3) years. He would like an inspector the inspect the well and see if it needs to be plugged. The lease road is a shared residential/agricultural drive that is stable. The well is an electrically driven pumpjack well with proper identification. The flowlines for the well are currently shut in. The last commercial production for this well was in 2012 when the well produced 873MCF of gas. The tank battery for the well is shared with well #25202 and contains two (2) 210bbl production tanks, two (2) vertical separators, a chart box, a concrete brine vault and house gas connection. The identification is faded on the tank battery. The complainant would like the well put into production, if that is not possible he would like the well plugged. I contacted Kyle Ahbel of Enervest and they are going to move this well to the top of the list for a workover. If there is no sign of progress on the well being put into production within 30 days, or the well not being commercially viable a compliance notice to plug the well will be issued. Enervest has been instructed to contact me to demonstrate that the well can produce when the work is complete and to update the identification on the tank battery.

1485876164 2016-01-23 Temporary Abandoned Annular Disposal No Violations I conducted Temporary Abandoned Annular Disposal (TAAD) status check inspection on this well. There is no annular disposal in use at this well. The well is equipped with a pumpjack with an electric motor. The diked tank battery consists of two 210 barrel tanks, two vertical separators, an inground concrete brine vault and a meter with a paper chart. The tank battery is shared with well # 151-25202 (Yoder #1). The well is legibly identified. There are no compliance issues noted at this time.
1734436389 2015-12-07 Production Wells No Violations I was on location to follow up to complaint #5172. A new flowline has been boreholed and installed to the tank battery for the well. A service rig is going to be coming on location to work on well and replace the downhole pump. When the work is completed and the well producing the complaint will be closed.
