0010252114 2014-11-12 Production Wells No Violations I was on location to follow up on complaint #4403. The well and tank battery have recently been worked over. A new dike, 100bbl production tank and separator have been installed where the existing tank battery stood. The lease road has been stabilized with several loads of stone and ceramic slag. A new chart has been installed in the chart box on 11-12-14 and shows that the well is producing. I have contacted PDL Services and they told me that the 2013 production reports are currently being submitted to the Division of Oil & Gas as required.
0050637718 2012-12-19 Production Wells No Violations I inspected the Direnzo-Fimble Unit #2 well (34151234030000) on December 19, 2012. My inspection showed the lease road was graded and top dressed with crushed brick aggregate to the tank battery and wellhead. The domestic gas line has been removed and is not connected to the well. The belts on the pumping unit have been replaced. The tubing and 4.5 inch casing valves still do not have bull plugs installed. The oil contamination surrounding the wellhead has been remediated and neutralized with lime. Fittings on the mechanical separator are still leaking. The dike is still overgrown with vegetation. The western side of the earthen berm has been repaired to prevent the loss of fluid from the site. I spoke to Jodi Carnahan and she said the pumping unit has been operational.

1393863090 2014-03-03 Production Wells No Violations During my inspection I found that the lease road was stable. A yellow plastic gas line is not buried and runs parallel to the lease road. The line has several segments that are damaged. The tubing and 4.5 inch casing valves do not have bull plugs installed and there are no pressure gauges installed. There is a small oil drip from the wellhead. There is no operator sign attached to the electrical box or at the wellhead. The tank battery consists of one 210 bbl stock tank and one vertical mechanical separator. The 210 bbl stock tank has the proper operator sign attached. There is a chart installed in the chart box with an installation date of 2-26-2014 indicating production. The dike is stable and is containing the spilled oil within it and at this time no oil was detected outside the dike. There are two oil drums inside the dike next to the separator leaking oil to a spill of approximately 10 by 5. There is a sound of liquid flowing into the 210 bbl tank. There is a oil spill next to the tank that differs in color from the oil leaking from the drums. The source of this oil could not be determined at this time due to the ice and snow in the dike. Issues for PDL Services Inc to address include; oil contamination around pump jack wellhead and inside the tank battery be cleaned up to include removal of contaminated soil, and the two leaking oil drums be removed. The yellow gas line along the lease road needs removed or repaired and properly buried. I have contacted the operator and been informed that the issues will be addressed this week.
1395866259 2014-03-26 Production Wells No Violations During my inspection I found that the lease road was stable. The plastic yellow gas line has been removed. The small oil drip from the wellhead has been fixed. The tank battery consists of one 210 bbl stock tank and one vertical mechanical separator. The 210 bbl stock tank has the proper operator sign attached. The dike is stable and the spilled oil within it has been cleaned up and removed. The two oil drums have been removed from the location and are no longer present. The oil spill near the drums has been cleaned up. The compliance notice for this well regarding contamination is now closed.
1486558553 2017-02-06 Production Wells No Violations I conducted a complaint follow-up inspection on this well for complaint #4403. The well is equipped with pumjack with an electric motor and is producing. The annual production report for 2015 has not been submitted. The diked tank battery consists of one 100 barrel tank, a vertical separator, and a meter with a paper chart. The well is legibly identified. I called and left a message with the complainant. I advised them the well is producing and the complaint will be closed.
