0136803323 2016-03-03 UrbanProduction Wells No Violations I conducted an inspection of this well as requested by the landowner Jan Weisel. The landowner was inquiring about the well production and had indicated that they no longer received any domestic gas from this well. The state database shows the last production record was in 2013. The wellhead had a pump jack on it and the valves were open. There was a domestic line with a meter and regulator plumbed into the 8 and 5/8 inch casing. The signage at the well was clear and legible. The tank battery consisted of a 200 BBL production tank, a 100 BBL production tank, heater treater separator, and a domestic line with meter and two drip bottles. The tank battery was diked. The lease access for the wellhead and tank battery was stable and driveable. I spoke with Don Geitgey from G D Oil & Gas and he indicated that the well was going to be plugged this summer of 2016. No violations at this time.
0429698308 2019-03-27 Production Wells No Violations I received an email from Adam Schroeder on 03-21-2019 advising me a Mrs. Janet Weisel had contacted him with concerns about this well. My inspection found the well is equipped with a pumpjack and the well is shut-in. There is a domestic gas connection at the well with the valve in the off position. The tank battery consists of a steel production tank, a steel brine tank and two vertical separators. The is a domestic gas connection at the tank battery with an open valve. The well had zero production for 2017. I phoned Mrs. Weisel (330-492-7510) and left a message. I emailed Adam Schroeder to advise him I inspected the well and attempted to contact Mrs. Weisel.
0820675640 2013-01-11 UrbanProduction Wells No Violations I inspected the Weisel R & E 1 (34151236500000) well on January 11, 2013. My inspection was a follow up for an existing Notice of Violation (NOV) issued by inspector Dave Claus on August 8, 2011. No operator identification sign posted at wellhead and the well was causing pollution and contamination. The leak and associated contamination were resolved on August 11, 2011. There is an operator identification sign posted at the site but the sign is not populated with required information. I called Don Geitgey to discuss posting the proper sign.

The leak was occurring at or near the stuffing box. The emergency phone number listed for this operator goes to a home answering machine. I contacted Woody McDaniel with Enervest for information since they operate most of the wells in the vicinity. He reported that the well was not theirs but he would send Joe Teppert to assist. I arrived on site just after Joe Teppert and Don Getchie, the well owner (cell 330-353-0834). The pump jack was running but there was no brine spraying into the air. There was a relatively small leak that was corrected by tightening the stuffing box. The contract pumper for this well is Bob Ditty (cell 330-495-1631). I directed Don Getchie to 1) replace the identification at the wellhead with a legible version, 2) add a 24 hour emergency phone number that does not go to an answering machine and 3) to remove, properly dispose and replace oil stained stone around the wellhead.
1202738067 2015-09-15 Production Wells No Violations I was on location to follow up on a compliance notice issued for improper identification. The lease road is stable and leads up to an electrically driven pumpjack. The electric box has an identification sign on it with updated the correct information. At this time the compliance notice for identification has been closed and there are no violations at this time.
