0250795184 2015-10-26 Production Wells Violations Noted I was on location to perform an inspection of this well following a report from an Enervest pumper that the 210bbl production tank has overflowed. Approximately 1bbl of oil has overflowed from the tank into the dike and did not leave the dike. It has been absorbed by mulch that is present in the dike. The tank has been emptied and there are absorbent pads and oil present on top of the tank. I have attempted to call the operator using both emergency phone numbers but only received the voicemail. I left a message indicating cleanup in the tank battery is required immediately and to return my phone call. The operator was also informed that production has not been submitted for the well since 2012 and that production must be submitted as soon as possible.
0414049457 2013-12-16 Urban Drill / Deepen / Reopen No Violations I was on location to check on a reported oil spill on a tank in this shared tank battery with the Mountcastle #1 well. The lease road is well maintained and the tank battery and the pumpjack well has the proper signage. The tank battery contains 3 210 barrel production tanks and a 50 barrel brine tank. It also contains 2 horizontal and 2 vertical separators. A call was received by Inspector Roush from Enervest explaining that the well owner went on vacation and left his pumpjack on, causing the well production tank to overflow. During my inspection I saw a slight crude oil sheen on the side of the tank battery and a 12x 10 spot on in the tank battery were the oil spilled on the ground. The spill has been cleaned up and the tank battery has also been cleaned up of spilled crude oil. There are no violations at this well during my inspection and the spill has been remediated at this time.
0536331508 2014-05-29 Production Wells Violations Noted I received a call from Enervest, who has a shared tank battery with this operator, stating that a 210 bbl production tank is overflowing with oil and that there is oil on the ground. During my inspection I found the side of the 210 bbl production tank and ladder to be covered in oil. Inside the dike there is approximately 2 barrels of oil on the ground. There is a small amount of oil that has leaked out of the dike to some grass on the backside of the tank battery. I have called the operator and he stated that he did not empty the production tank before he pumped the well, causing the tank overflow. He has called J.Hawk services to come out to location to pump down the tank and clean up the spill. The oil needs to be cleaned up and the contaminated soil removed. The small amount of oil in the grass needs to be removed. I will be following up on this June 1st to confirm the clean up has occurred.
0651751069 2015-11-02 Production Wells No Violations I was on location to check the status of the 1bbl oil spill that occurred on 10/26/2015. The spill occurred into the dike of the tank battery for this well. Approximately 1bbl of oil has overflowed from the tank into the dike and did not leave the dike. It has been absorbed by mulch that is present in the dike, the mulch has been removed from the dike and disposed of. The oil present on top of the 210bbl tank has been cleaned up and the absorbent pads have been disposed of. A Jhawk water truck sucked the dike and tanks empty. The operator has obtained a Form 10 to submit annual production. The form has been mailed to ODNR via registered mail. The cleanup has been completed at this time. When the production is received by the state the compliance notice will be closed.
1402409674 2014-06-10 Production Wells Violations Noted I was on location to follow up to a spill that occurred on 5/29. Clean up has been completed and there are oil soaked adsorbent pads on location ready for disposal in a trash can next to the tank battery. The production tank has been cleaned up and has had the proper I.D. signage installed on it. I have contacted the operator and he has told me he is throwing away the oil pads today. At this time the compliance notice for contamination and I.D. signage has been passed.

The saturated mulch inside of the dike has partially been removed from the dike. A few small pockets of oil are still present inside the dike. A Jhawk water truck is going to be on location to suck the dike and tanks empty. The operator is going to bring a mini hoe onto location during the weekend to remove any remaining saturated mulch. The operator has also obtained a Form 10 to submit annual production. The form has been mailed to ODNR via registered mail. I will perform a follow up inspection on 11/2/2015 to ensure the clean up is completed.
