0022479433 2002-08-02 Production Wells No Violations CASE 02-017 - Salem office received a complaint from Jeff Weisel regarding a leaking pipeline at the Seifert farm. The pipeline was bubbling gas. Belden & Blake owns the Seifert #2 well and Petrox owns the Serifert #1 well. Supr. Simmers responded to the complaint and notified Supr. Cheslock and Insp. Worstall of the location and scope of the leak (approximately 24 square feet) and the well involved (#2). Belden & Blake sent their pumper to shut the well in. Ted Lauver w/ Belden & Blake contacted Arrow Oilfield Service to send a roustabout crew to replace the leaking flowline. Insp. Worstall stayed on site to witness the replacement of a 12 section of flowine. After replacement, the line was pressurized with no leaks detected. The open trench will be closed in and the impacted area reclaimed.
0036079008 2002-08-09 Production Wells No Violations Pipeline has been repaired. Trench was backfilled and graded level. VIOLATION RESOLVED.
0254343365 2015-01-02 Production Wells No Violations I was contacted by Enervest stating that there has been a flow line leak at this well. A total of approximately 1bbl of oil has been spilled. As I arrived on location Jerrys Excavating had cleaned up all the spilled oil, the flow line had been patched and pressure tested. The cleanup crew is awaiting an electrician to arrive on location to fix the power lines to the pumpjack that were dug up on accident when the cleanup was occurring. The excavated soil has been loaded to be disposed of at an EPA approved landfill, and when the power lines are fixed new soil will be replaced where the spill occurred. I will follow up on 1/5/15 to ensure the cleanup has occurred.
0305570972 2014-07-08 Production Wells No Violations I was on location to follow up on this wells tank battery that was struck by lightning. A new 210bbl production tank has been installed and new fittings and pipes have been installed on the vertical separator. The gravel in the tank battery and the lease road has been scraped up and replaced. The spill and damage done by the lightning has been cleaned up and the well is back into production. There are no violations at this time.
0327448442 2015-01-05 Production Wells No Violations I was on location to follow up to complaint #4903 and the oil spill that occurred on the flow line for this well. The approximately 1 bbl of oil that leaked from the flow line has been cleaned up and the flow line repaired and pressure tested. The electric lines that were accidentally dug up during remediation have been repaired and buried. The cleanup for this well has been completed and the complaint can now be closed.
2145485813 2014-06-29 Production Wells No Violations I was called by Doug Beighle of Enervest (330-365-7079) stating that the production tank for this well was struck by lightning and was on fire. I arrived when the fire was put out by the Marlboro Fire Department. They used 5 gallons of foam at a 1% concentration. The tank battery is shared and contains three 210bbl production tanks and three separators. The pumpjack wellhead has been shut in and the other two wells flowing to this tank battery have also been shut in. The explosion from the lightning blew the vent stack about 50 feet into a field near by and also blew off the top of the tank which landed nearby. The separator for the well nearby was damaged and bent from the blast. A total of about 10 gallons of oil left the dike and is outside of the dike. The 210bbl tank has been pumped down to another tank. Peat sorb and adsorbent pads have been used to adsorb the oil inside the dike and the lease road next to the tank. There is still a large quantity of fire department foam on location. The spill is contained and the oil has been cleaned up. Work will continue tomorrow morning to clean up any stained rock and oil. Damaged equipment will be removed and new equipment installed. I will follow up on this tomorrow.
