| 1649182693 | 2019-04-30 | Production Wells | Violations Noted | My inspection found the well is equipped with a casing head with a closed valve. There is no plug in the casing head. The wells last reported production was in 2001 for 75 barrels of oil. The Annual Production Reports from 2010 through 2018 show zero production. There are no Annual Production Reports for 2002 through 2009. The tank battery consists of a 210-barrel production tank, an inground concrete brine vault and a meter. The tank battery is overgrown with vegetation. The well is not legibly identified. I called John Raber (M & R) on 05-01-2019, and I left a message advising a Compliance Notice to plug the well was being issued. The well is Idle. Consequently, the well is in violation of Revised code 1509.062 and shall be plugged in accordance with Ohio Revised Code section 1509.12 (B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
