A pit was installed for any fluids encountered during the plugging process. The wellhead valves were opened up and no pressure was noted on the 4.5 inch casing, however there was gas present from the 8 and 5/8 inch casing. I witnessed the rods and tubing removed from the well casing prior to plugging activities. The site is stable and vegetated and access to the wellhead is stable and driveable. |

The plug was set through 2 and 3/8 inch tubing due to a possible hole in the casing. Bishop Well Service placed the tubing at 4000 feet to set the bottom plug. Bottom plug was set from 4000 to 3642 feet. Petroset set plug using 2% gel and the weighed cement was 15.5 ppg. After plug was set, tubing was tripped out of the hole to allow the cement to set and before attempting to rip pipe. Bishop tried to rip pipe but could not get past 1000 feet downhole due to obstruction. Appalachian Well Service arrived on site to shoot the 4.5 inch casing to proceed with next plugs. The 4.5 inch casing was shot at 2621 feet. Will come back next day to proceed with plugging. |

The well had been shut in overnight and pressure at the wellhead was 30 psi this morning. Bishop rigged up and removed the 4.5 inch casing completely from the wellbore and the 2 3/8 inch tubing was tripped back in to set the Big Lime plug. Petroset broke circulation prior to setting the Big Lime plug which was then set from 2599 to 2299 feet. During circulation oil and gas were present. The weighed cement was 14.5 ppg and had an addition of 2% gel. Bishop pulled 82 joints of tubing to set the surface plug. Petroset rigged up and broke circulation and set the surface plug from 748 to 458 feet. During circulation oil and gas were present. The cement weighed 14.4 ppg. The decision was made by Enervest to grout the top of the well with 9 sack concrete. The top plug will be set at a later date. |

The electrically driven pumpjack well is shut in, surrounded by a fence and has had the electric motor removed. The tank battery is surrounded by a dike and identified. The tank battery contains a 210bbl proudction tank. The separator has been removed from the tank battery. The last production for this well was in 2010 when it produced 927MCF of gas. This well is currently not capable of production. The well owner/operator shall submit a written plugging schedule to Fred Romeo at 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
