1371808432 | 2015-11-24 | Production Wells | No Violations | I was on location to perform an inspection of this production well. The lease road is gravel and a shared residential driveway. The tank battery is surrounded by a dike and identified. The tank battery contains a 210bbl production tank, concrete brine vault, vertical separator and chart box. The electrically driven pumpjack has a house gas connection and an area of oil staining around the wellhead, and the wellhead is covered in oil. There is a concerete brine vault half way between the wellhead and the lease road entrace. The hose running to the brine vault has many holes in it. I contacted Steve Sigler of Buckeye stating that the oil staining and wellhead must be cleaned up and it discontinue use of the concrete brine vault and to remove the hose from the location. |
1479148838 | 2016-11-10 | Production Wells | No Violations | I received a phone call at 7:43AM from Scott King (Oil & Gas duty line officer) about a gas line leak in Stark County located off Of Gracemont Rd. The gas leak was reported by a Division of Wildlife employee who had been doing a wildlife study on mink in the early morning hours. After arriving on location I contacted Tim McConaha (Enervest) and advised him Enervest may have a potential gas leak on one of their lines. Jay Jones (Enervest Pumper) arrived on location and we were able to find the leak. The leak was located at (40.66452, -81.42496). Initially the leaking line was thought to be an Enverest well, the pumper shut the well in and the gas leak did not slow done. Tim McConaha arrived on location and contacted the Enervest pipeline crew and they thought it may belong to Aspire or Buckeye Oil. Tim McConaha contacted John Huddleson (Aspire) about the leaking gas line he advised Tim McConaha that Aspire did not have a gas lines in this particular area. I notified Ron Maurer (Buckeye Oil Field Superintendent) he advised me he was familiar with the line and it was in fact Buckeye Oils line. Mr. Maurer arrived on location and I showed him where the leak was located. He left for ~ 1/2 hr in order to turn-off valves to isolate the line. The line is a 3 collection line for four well located west of I-77. I remained on location until the gas pressure on the line bled off. Mr. Maurer advised me he would do the repairs on Friday November 11, 2016 (Veterans Day). I contacted Scott King about the repair schedule and he advised me it would be good. I will follow-up on Monday November 14, 2016 to check repairs. |
1479152902 | 2016-11-14 | Production Wells | No Violations | I conducted a status check inspection a gas line leak reported on 11-10-2016 had been repaired. My inspection found the repaired had been made and the collection line was not leaking. I spoke with Ron Maurer (Field Superintendent) and he said it was pressure tested prior to being placed back into service. |
