0080886062 2014-05-06 Production Wells No Violations I was contacted at 11:30am by Doug (330-365-7079), the Enervest Manager, regarding a flow line leak that occurred near the wellhead. I was on location with Inspector Jim Bucksar. Also on site was Joe Little Excavating cleaning up the spill. The affected area was a 120 by 30 area of the plowed field with a small pooling of crude oil on the surface. It is estimated the spill was a total of 4bbls. The affected topsoil was scraped up and put inside of a liner and covered with a tarp on the lease road to ensure it does not erode or leave location. A Kimble Company dumpster will be on location tomorrow to haul away the contaminated soil. The soil has been stabilized with peat sorb and is covered to prevent any rain from affecting the contaminated soil before it will be moved off location. I witnessed the digging up of the flow line. A small pinhole leak was located. The leak was patched, and the flow line pressure tested to ensure there were no more leaks. The flow line was then covered with soil and the area graded. Field grade topsoil is going to be brought on location to cover are the area where the contaminated soil was removed. I will follow up to ensure the topsoil was put back onto the field, that the contaminated soil will be properly disposed, and that no more oil is present on location.
0811952451 2014-05-12 Production Wells No Violations I was on location to follow-up to a spill that occurred May 6th. The oil has been remediated and the well has been cleaned up. A flow line patch was used and pressure tested to ensure no more leaks were present in the line. The contaminated soil has been removed off location by Kimble Companies and will be brought to the landfill and proper disposed of. At this time the spill has been cleaned up and the production equipment brought back into working condition properly. There are no violations at this time.
