0151876072 2013-01-09 Production Wells No Violations I inspected the G & G Straub 6 (34151239850000) well on January 9, 2013. My inspection was a follow up for an existing Notice of Violation (NOV) issued by inspector Dave Claus on February 13, 2004. All valves are closed. The well is idle and has not produced since 1985. I called the number posted in RBDMS and the number is not in service.
0223880351 2004-02-13 Production Wells No Violations FOLLOW UP TO NOV # 1076694241 The well is swedged from 4 to 2. The ball valve on the swedge is closed and the ball valve on the 2 line is closed. There is one heavily rusted 210 bbl stock tank and a rusted horizontal separator (both shared with permit #3679). There was no brine vault or tank observed. The I.D. sign on the stock tank was partially visible. The well appears to be idle. RBDMS shows the last reported production was in 1984. There is good access to the tank battery and to the wellhead. GPS and photographed the WH and the TB. Place a new I.D. sign on the stock tank. Plug or produce the well.

1066251962 2013-06-20 Production Wells Violations Noted I arrived on location and made a complete inspection of this permit, my findings are as follows; Well is idle and storage tanks have been removed, wellhead is covered by brush and the lease road no longer exists. I checked production reports and found they have not been submitted since 1984. Plugging is required within 30 days. Failure to comply may result in forfeiture of bond.

This is a swab well with a 4 inch swage reducing to 2 inches. The tank battery consist of steel production tank which is lying over on its side and a horizontal separator. Both the well and tank battery are overgrown with vegetation. The well and the production equipment is heavily rusted.

Doug Gonalez (Plugging Consultant), Bishop Well Services (Plugging Contractor), Petroset (Cement Contractor) and S & H oil field services (water trucks) are on location to set the bottom plug. Fluid level is ~ 600 ft. in the hole.
1440133465 2016-12-15 Production Wells No Violations I conducted a compliance notice follow-up inspection on this well. The well is equipped with a swage and a gauge that has zero pressure on it. There are no annual production reports submitted since 1984. The well is not legibly identified and no Plug & Abandon permit has been issued. This well has had two compliance notices issued on it to plug the well. The first one issued by inspector Dave Claus issued on 2/13/2004 and the other issued by inspector Barry Grimm on 6/20/2013. None of the compliance issues cited in the compliance notices have been complied with. I called the number list as the record 304-233--73 and 2014-74 which were issued 03-31-2014 for Spring Industry/ Straub wells located in Tuscarawas County.

1516719077 2018-01-23 Production Wells No Violations I did a compliance follow-up inspection for this well. The well is not plugged. Kelly Daley (Daley Excavating) is on location grubbing in an access road and prepping the well location for plugging. The two steel production tanks and a vertical separator have been removed. A horizontal separator and some flowline remains.
1516816214 2018-01-24 Plug / Plug Back No Violations I was on location with Shawn Murphy, for a training exercise, during the setting of the Clinton sand plug. Bishop Well Service (BWS) was the plugging contractor and Petroset Cementing Services was the cement contractor. After removing the 2.375 tubing from the 4.5 casing, BWS tagged the fluid level at 600. Well was secured and shut in overnight.
1517242185 2018-01-29 Plug / Plug Back No Violations I am on location with Bob Roush (MRI-3) to witness the plugging of this well. The 4.5 inch casing was ripped at 3156 Ft. There was a small kick and the annulus began to flow once the rip was completed. The hole was circulated until the fluid is clear.
1517414288 2018-01-30 Plug / Plug Back No Violations I witnessed the plugging of this well with Bob Roush (MRI-3). The bottom and top surface plugs were set and the cement top is ~ 3 ft below the top of the casing. The well is plugged and the Compliance Notice is passed.

1525798115 2018-05-08 Final Restoration No Violations I did a Final Restoration inspection for this well. The well was plugged on 01-30-2018 and a plugging report is on file in the wells record. A landowner waiver was received at the Uniontown office of Oil & Gas on 05-08-2018. The landowner waiver waives the establishment of vegetative cover at the wellsite, the backfilling and grading of the access road and the removal of production equipment - buried production line. The well and tank battery sites are graded with no signs of active erosion. There is no visual evidence of the well or tank battery. The Final Restoration is passed.

1750197816 2016-05-17 Production Wells No Violations I conducted a compliance notice follow-up inspection on this site. The wellhead consisted of a 4.5 inch casing with Swage and tapped plug with a gauge. No pressure was showing on the gauge at time of inspection. A 2 inch flowline off of the 4.5 inch casing was shut-in. There was no signage at the wellhead. The tank battery on site consisted of two, 210 BBL production tanks and two mechanical separators. Both 210 BBL production tanks were disconnected from dump lines and turned on their sides. The tank battery was not diked. The lease access was driveable. No production records have been submitted since 1984. No changes since the last report inspected on 6/20/2013.
