0645286634 2017-10-30 Final Restoration No Violations I did a final restoration inspection for this well. There is no visual sign of the well or tank battery. The final restoration cannot be passed at this time. There are two power poles on the lease road going back to the well which need to be removed or a landowner waiver waiving the removal of the power poles. A landowner waiver is needed for the flowlines to remain. An email was sent to Duane Clark (Petrox) on 10-30-2017.
1353501231 2016-03-29 Production Wells No Violations I conducted a compliance notice follow-up inspection of this well. As was found during the previous inspection on 1/27/16, the well has had the pump jack removed. There is a 210 BBL production tank sitting adjacent to the well with no dike around it. Signage on the tank corresponds to the well API number. The tank battery has had the two, 210 BBL production tanks removed along with the concrete brine vault. The oil stained soil around the base of the production tanks has been cleaned up and removed. Proof of proper disposal must be submitted to Inspector Mullins at the address listed below. There is also a domestic line with a meter, drip bottle and chart boxes which has been left onsite inside the dike. No new signage has been added to the remaining tank battery equipment or wells. The tank battery is shared with well #22948. The lease access is stable and driveable. A Compliance Notice was issued to the operator on 3/11/15 requiring the well to be plugged within 180 days (9/11/15), as the well was not capable of production, and had not had production reported since 2008. A Permit to Plug and Abandon the well was issued to the owner by the Division of Oil and Gas Resources Management, dated May 18, 2015, but the well has not yet been plugged. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days, per OAC 1501:9-11-05(B). The well owner/operator will be notified if the plugging schedule is approved or denied. In the interim, the owner must correct the identification sign.
1455110000 2016-01-27 Production Wells Violations Noted I conducted a compliance notice follow-up inspection of this well. As was found during the previous inspection on 3/6/15, the well has had the pump jack removed. There is a 210 BBL production tank sitting adjacent to the well with no dike around it. Signage on the tank corresponds to the well API number. The tank battery consisted of two 210 BBL production tanks, vertical separator and concrete brine vault. There is also a domestic line with a meter and drip bottle. The chart boxes were empty. The 210 BBL tank is leaking at the base and at the heater tube. The concrete brine vault needs emptied. The tank battery is shared with well #22948. There is signage at the tank battery located on the 210 BBL but needs updated to reflect corrrect phone number. The lease access is stable and driveable. I spoke with Scott Dutton of Petrox, Inc. and he said that will contact me when he finds out what future plans are for the well. A Compliance Notice was issued to the operator on 3/11/15 requiring the well to be plugged within 180 days (9/11/15), as the well was not capable of production, and had not had production reported since 2008. 1481030366 2016-11-14 Production Wells No Violations I conducted a status check inspection on this well prior to the start of plugging operations. The pumpjack unit has been removed. The rods and tubing are still in place. The equipment with in the tank battery has been removed. The tank battery is shared with well # 151-22948 which will also be plugged.
1510758205 2017-11-15 Final Restoration No Violations I did a final restoration inspection for this well. The well site and tank battery site are graded and vegetated with no active erosion. A landowner waiver was received on 11-14-2017 at the Uniontown office of Oil & Gas waiving the removal of the flowline. I reviewed satellite imagery for power poles and power line going back to the well. I did not see anything resembling any electrical power going back to the well site. With the receipt of the landowner waiver, the Final Restoration is passed

The tank battery contains two (2) 210bbl production tanks, vertical separator and concrete brine vault. The tank battery is shared with well #22948. This well has not produced since 2008. This well is incapable of production and shall be plugged per Ohio Revised Code 1509.12(B) within 180 days.
