1154620819 2006-08-02 Production Wells Violations Noted I received a call from Jim Irwin (cell # 216-789-9281) of the OEPA at 1145 hrs. regarding a flow line leak and a crude oil release into Nimishillen Creek. I inspected the site and met with Jim Irwin (OEPA), Don McDonald (cell # 330-714-6609, Stark County Local Emergency Planning Committee), Fred Klingaman (cell # 330-704-2377, Plain Township Fire Department), Tim McConaha (cell # 330-903-4782, Belden & Blake) and Troy Wells (cell # 330-571-2310, Belden & Blake). I also received a call from Rachelle King (304-539-3029, EnerVest (Belden & Blakes parent company) informing me of the release. The release had been called into the Plain Twp. Fire Department and to the OEPA on the evening of 8-1-06. The Plain Twp. Fire Department had responded and placed an absorbent oil boom downstream of the release area at approximately 300 south of the Fromes Ave. and Diamond St. bridge. At my arrival on 8-2-06, Belden & Blake personnel were placing additional booms and absorbent pads in the creek, and excavating the oil saturated soil. The release area on the ground (GPS 40.90599, 81.30505) measured 100 long by 15 wide and was 10 from the edge of Nimishillen Creek. The crude oil leached through the soil and was seeping into Nimishillen Creek. Approximately 4400 (.83 miles) of the creek was affected. A nearby property owner, Bob Masters (330-933-1409) at 3680 Diamond St, had 20 adult cattle and 20 calves that may have drank from the creek. He will watch his cattle for any adverse effects. I estimated the release (on the ground and on the creek) to be less than 2 barrels of crude oil. On 8-3-06, I inspected all the oil booms and met with Belden & Blake personnel. Belden & Blakes personnel will be wading downstream from the release area and sweeping out any pockets of crude oil so that the oil can be caught in the booms located downstream. All of the oil saturated soil will be excavated out today and trucked to American Landfill for disposal. Belden & Blake will replace the flow line fro
