1538739959 2018-09-29 Production Wells Violations Noted I received phone calls from Kyle Fink (Enervest) and Fred Romeo (Emergency Response - Oil & Gas) concerning and tank battery overflow in which oil got into a tributary of the East Branch Nimishillen Creek. The estimated spill was two barrels of oil. When I arrived on location I met with Mike Burkhart (Enervest) and Kyle Fink. The oil in the creek was contained using booms and a siphon dam and clean-up operation were underway. The leak into the creek started at the tank battery. There was no sign of overflow of the containment dike. The oil entered the stream by perking down through the steam bank soil - Leak location +/- 9 (40.84740, -81.20991). The estimated stream effacement is 600 Ft. The reported cause of the overflow was a timer at the well malfunctioned. I inspected the well and the electrical switch was in the OFF position. Repair well components as needed so the timer functions as designed. Ensure the tank battery is cleaned-up and repaired so any future leaks are contained in the dike. Continue with downstream oil mitigation until such a time there is no evidence of oil in the stream.
1575680603 2018-10-04 Production Wells No Violations I met with Kyle Fink (Enervest) and Tim McConaha (Enervest) on location. Clean-up is still progressing. I did not notice any more oil purging from the stream bank below the tank battery.
2146257413 2018-11-23 Production Wells No Violations My inspection found no sign of oil in the stream or on the stream bank. The siphon dam and down stream booms have been removed. The Compliance Notice issued on 09-29-2018 is Passed.
