0079508913 | 2009-08-03 | Final Restoration | No Violations | This well was plugged and abandoned on 7-6-2009. The site has not been reclaimed. The 8-5/8 dia wellhead, pump jack pad and sills, and the production stock tank are still in place. I did not pass the Final Restoration at this time. |
0562998346 | 2017-06-22 | Final Restoration | No Violations | I conducted a Final Restoration inspection on this well. The ladder and flowline have been removed from the tank battery which is noted in my previous inspection report dated 4-20-2017. The tank battery was shared with producing well # 151-24073. The well location is covered with a grain crop and there are no signs of active erosion. The Final Restoration is passed. |
1161900441 | 2006-10-26 | Production Wells | No Violations | Met with Jim Hawkins on location to discuss a potential UIC facility. |
1387201142 | 2013-08-20 | Final Restoration | No Violations | There was no indication of the well head, or lease road. The tank batteries were still on location and there was no indication of another well flowing into them. Final restoration does not pass. |

1493651107 | 2017-04-20 | Final Restoration | No Violations | I conducted a Final Restoration inspection on this well. The well site is graded and vegetated with no signs of erosion. The tank battery is shared with Enervest well # 151-24073 (Landis #1). I spoke with Tim McConaha (Enervest) he advised me the inground concrete brine vault belongs to the Landis #1 well. The tank battery equipment has been removed with the exception of some flowline and the ladder to the stock tank which are within the tank battery dike. I called and spoke with Brian Levengood he advised me he would get the ladder and flowline removed so the Final Restoration can be passed. |
2040811498 | 2006-10-16 | Production Wells | No Violations | I inspected this well at the request of Tom Tomastik. The well is being considered for conversion to a salt water injection well. The access to the site is thru a gravel residential driveway off of Justus Rd. The nearest structure to the wellhead is a horse barn and an associated fenced pasture located 130 to the north. There were no surface waters or drainage waterways observed within 500 of the wellhead. There were no violations at the wellhead or at the tank battery (1-210 bbl oil stock tank, 1-shared concrete brine vault) at the time of the inspection. There is an existing rabbit well (Landis # 1 - Permit # 4073) located approximately 300 to the SE of the Landis # 2 wellhead. The Landis # 1 and the Landis # 2 stock tanks are next to each other and the 2 wells share a brine vault. There are several newer homes located along Crestline Rd. approximately 700 to the south of the wellhead. There is a newer home located more than 500 to the NE and down gradient of the wellhead. There are also homes located to the west of the wellhead along Justus Rd. I did not observe any water well casings sticking up, but I assume that all the homes are on well water. Since there are newer homes in the area, I suggest that Dover-Atwood Corporation submit a recent aerial photo of the site showing the location of the proposed brine holding area and associated pump house. The area is quiet, with families and horses in the immediate area. Truck traffic, pump noise, and the odor of crude oil, could result in public complaints to the facility and need to be considered in the evaluation process. |
