1229629293 2008-12-17 Production Wells Violations Noted I received a call from Chris Holmes (OEPA - Columbus) regarding an anonymous report of pollution and contamination at this well site. I met with Bart Ray (OEPA) at the site and discussed the situation. J-Wells service rig is on the location doing a pump change. There is surface staining at the site and saw dust has been put down to absorb liquid. There is no evidence that the liquid reached the storm drain. I called George Strawn and told him to clean up the site when the service rig moves off of the location.
1283026933 2009-01-09 Production Wells No Violations The surface release appears to be cleaned up. However, due to the snow cover, it cannot be determined if the N.O.V. has been completely resolved. I will inspect the site again when the snow cover is gone.
