The lease road is in good condition, graveled, and does not show signs of erosion. The tank battery consists of one 210 bbl stock tank, a poly brine tank, and two mechanical separators. There is a chart at the site but it does not indicate an installation date and has no activity recorded. The saltwater discharge line connected to the 210 bbl stock tank and the ploy brine tank is corroded but is not leaking any brine or fluids on the ground inside the dike. All other fittings, connections, and valves show moderate to high levels of corrosion but not a loss of structural integrity. The 210 bbl stock tank has the proper identification sign attached. The dike has good walls and containment capacity but has a moderate amount of vegetation growing inside. The drain line and the brine discharge line on the 210bbl stock tank are closed and capped to prevent an uncontrolled release. The previous compliance notice of dripping fluid from poly brine tank has been addressed and the compliance notice can be closed. The adjacent landowner stopped me while on location and explained to me that the pump jack has been removed from the well and that the well has been idle since the summer. He stated that an Enervest representative told him that the well will be plugged, while another Enervest representative said that the well will be swabbed and put back into production. This well is currently idle and unable to produce. Enervest has been contacted regarding this well and it is going to be plugged and abandoned this summer (2014). The complaint can now be closed. |

Chipco began rigging down to move. The circulating pit was emptied and closed and I approved cutting off the 8.625 surface casing. The cement plug was holding in the hole two (2) feet below grade and the ID plate was attached. See plugging report for full details on this job. No violations. |

Chipco Rig #6 has spotted tubing at a depth of 7571 just above the bottom perforation at 7589. Chipco rigged up to the tubing, loaded the hole, and broke circulation. They then mixed and pumped 6.3 barrels of cement slurry and displaced the slurry to 7184 with 28 barrels of water. The tubing was pulled above the cement, reverse circulated, and the well was shut-in. No violations. |

The tubing valve was opened and the well was static inside the 4.5 production casing. The Oriskany had charged up the annulus between the 4.5 production casing and the 8.625 surface casing while shut-in overnight and gas was leaking from the 8.625 wellhead slips. I had Chipco extend a 2 steel line from the 8.625 annular valve down the lined flow ditch and attach a 90 degree elbow directed to the bottom of the circulating pit. The base of the line was supported by timbers and I had them place the dozer blade on top of the line for safety. The annular valve was opened and the backside unloaded gas and saltwater for five (5) minutes. Chipco pulled the remainder of the tubing from the hole and switched out trailers. They ran in the hole with the knife tool and ripped the 4.5 production casing at a depth of 3324 and pulled one (1) joint of casing to 3283 (40 below the Oriskany) to set the Big Lime plug. Chipco rigged up to the casing and began circulating the hole. They pumped 90 barrels of freshwater and the Oriskany continued to unload pockets of gas and saltwater. At this point, I had Chipco start circulating the hole with the heavier, produced salt water from the pit. After 1.5 hours of circulating with salt water, static conditions were achieved and the Oriskany was no longer unloading or gassing. Chipco then mixed and pumped 18 barrels of cement slurry with 50lbs of cottonseed hulls and displaced the slurry to 2983 with 47 barrels of salt water. Circulation was good and never made a kick during this stage. Chipco pulled 19 joints (800) of casing, packed in the wellhead, and shut down for the day. No violations. |

Met with Sandy and C.B. Lester at their home. There was no odor in their house or in the running water. Pulled their water filter; no odor in the filter only a black residue that is not normally seen in the filter until recently. The Stark Co. Health Dept. will be sampling the water well on May 5th or 6th. I informed the Lesters that the odor is from the vent pipe on the stock tank. When the rabbit well cycles there is gas that vents from the stock tank. I contacted Dave Cumberledge 330-866-3301, cell # 330-495-3307, of Great Lakes Energy, and suggested that a charcoal filter be placed on the vent pipe. Dave agreed and said that Great Lakes had sampled the Lester water well prior to drilling the Hazen well. I will follow up the Lesters and the Health Dept. after the water well sampling results are completed. No violations were issued at this time. |

The Hazen 1-K (34-151-2-5291-00-00) is located on the right side of McCallum Avenue before Snode Street. I inspected the Hazen 1-K site on October 11, 2012. The lease road is in good condition, graveled, and does not show signs of erosion. The tank battery consists of one 210 bbl stock tank, a ploy brine tank, and two mechanical separators. There is a chart at the site but it does not indicate an installation date and has no activity recorded. The saltwater discharge line connected to the 210 bbl stock tank and the ploy brine tank is corroded and dripping saltwater on the ground inside the dike. All other fittings, connections, and valves show moderate to high levels of corrosion but not a loss of structural integrity. The 210 bbl stock tank has the proper identification sign attached. The dike has good walls and containment capacity but has a moderate amount of vegetation growing inside. The drain line and the brine discharge line on the 210bbl stock tank are closed and capped to prevent an uncontrolled release but are not locked. All valves on the flow lines from the wellhead to the tank battery are open, indicating the well is not idle. There are no unburied flow lines between the wellhead and tank battery. The wellhead does not show signs of oil, gas, or saltwater leaks at this time or any indication of past leakage. Electrical lines for the pumping unit motor are buried from the road to the wellhead. There are no restoration issues to report. At the time of inspection no odors were detected using a gas meter. The fencing surrounding the wellhead is in good condition but the gate is not locked. The wellhead/pumping unit has no identification sign. |

I met with Doug from Enervest Operating LLC to check the pressure on the 4.5 inch production casing and the annular space on the 8.625 inch casing. The pressure gauges on the 4.5 inch production casing and the production tubing both read 0 psi. Doug installed a new pressure gauge on the 4.5 inch casing and the gauge registered 2 psi. The valve for the 8.625 inch casing is buried and requires a small backhoe to remove the earth covering the valve. According to Doug, the well has been offline for approximately 8 months due to parted rods and 95% of the production is water. Doug will call on Monday with a time for excavating the 8.625 inch valve. The Hazen 1 (34-151-2-2528-00-00) site is approximately 377 feet northeast of the Hazen 1-K wellhead and was plugged in 1991. I inspected the area of the plugged well and lush green grass is growing where the plugged and abandoned well is located, indicating no issues. |

The complainant states that this well is not producing and has had equipment left on location from the drilling and servicing of the well. The lease road is gated and stable. of only a 4.5 diameter collar with a 2 swage and a shut valve. The flowlines have been disconnected from the well. The fencing around the well has been partially removed and is sitting next to the well. The tank battery is surrounded by a dike and contains identification. The tank battery contains a 210bbl production tank, poly brine tank and flow lines. The separator has been disconnected and removed from the tank battery. The last production for this well was in 2012 when it produced 188MCF of gas. This well is currently not capable of production. The well owner/operator shall submit a written plugging schedule to Fred Romeo at 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |

Alborn Excavating used a Bobcat to excavate the area around the wellhead to expose the 8.625 inch valve. Doug with Enervest Operating LLC attached a pressure gauge and opened the valve to record 250 psi in the annulus. I had Doug open the valve and bleed off the pressure. The well has 786 feet of 8.625 inch casing in the hole, which would allow for a maximum pressure of 238 psi. Enervest Operating will install a pop off valve on the 8.625 inch annulus, set at 25 psi to prevent a buildup of pressure. |

The 4.5 casing was perforated and plugs were set and squeezed under pressure to isolate the Utica/Point Pleasant interval and the Clinton Sandstone. The Oriskany Sandstone began unloading natural gas, oil, and salt water from the annular port on the 8.625 surface casing during plugging operations. The wellhead was packed in and the annular valve was closed until the following morning. Refer to the plugging report for details on the squeeze jobs. No violations. |
