1298662020 2017-12-08 Production Wells No Violations With Stephen Ochs (District Supervisor Oil & Gas): I did a status check inspection for this well. The well is equipped with a pumpjack. The tank battery consists of steel production tank, a steel brine tank, horizontal separator and a meter. The wells sign needs to be changed it is peeling away from the tank battery. I called and spoke with Bob Whaley (Enervest Foreman) and he advised me he would have the sign replaced.

1907214893 2006-03-17 Preliminary Restoration No Violations I inspected the tank battery and wellhead with Rick Simmers in response to a written complaint (gas odor from a tank battery and safety issues) received by Scott Oelslager that was forwarded to the Director of ODNR. This complaint was originally handled and resolved January 25, 2006. There is no violation of oil & gas law at this site. Belden & Blake has agreed to fence the wellhead and tank battery; place a belt guard on the pumping unit; clean up an empty drum and propane cylinder at the wellhead; lock the hatches on the stock tank and the brine tank; and close the opening in the top of the brine tank, all within 60 days for safety issues. A filter on the vent stack on the stock tank is already in place. I discussed these issues with Tim McConaha (Belden & Blake) on 1-25-06 and again today. Tim said that W. Glick (the property owner) will be reclaiming the site. I informed Wayne Schalk of my inspection findings.
