It was determined that when the well was being worked on the flow line cracked and has been leaking oil that was being pumped by additional wells in the area using a shared flow line. The surrounding wells have been shut in the and drain tile capped to stop the flow of oil. Additional equipment will be brought onto location tomorrow 1/21/2015 to finish the remediation work. |

The well pressured up to 1400psi and was shut in for 45min before the valve was opened. The well slightly flowed back, so the plug was shut in for an additional 30min. When the valve was opened again the well was dead and did not flow. The service rig ripped the 4.5 diameter casing at 3700 and pulled up to 3369 to set the Big Lime plug. When the casing was pulled to 3369 the Oriskany Sandstone kicked off and flowed 30bbls of oil. It was decided that the plug would not be set and a swab rig would be brought onto location to try to determine the economic viability of the Oriskany. A permit to plug back will be filed for if it is decided to produce the Oriskany. |

1418743027 | 2014-12-15 | Production Wells | Violations Noted | A phone call was received from the Stark County Health Department stating that people walking on the walking path near address 13281 Bayton St NE noticed oil coming out of a drain tile pipe into a ditch. I contacted Greg New and Paul Shaw and informed of the spill occurring from this well. The Washington Twn fire department was on location and installed adsorbent pads and booms to control the spill. Ron Fodo of the Ohio EPA was called and informed of the spill. Sweet Meadow Farm Drainage was the contractor called to perform the cleanup. It was determined that when 4.5 diameter casing was ripped to pull it from the well during plugging operations, the Oriskany kicked off and flowed oil. Approximately 5 gallons of oil were not cleaned up and entered the drain tile. After the recent rain the oil flowed to the ditch. An area approximately 30x3 in the ditch was impacted. The banks of ditch were not affected. There is no free oil on the surface or visible near the wellhead. All visible free oil was cleaned up and a boom was installed near the drain tile outflow and at the end of the ditch. A bag of adsorbent pads were put in the ditch to absorb any free oil. In the morning a water truck will be brought onto location to flush the drain tile and remove any oil from the pipe. The oil must be fully flushed from the drain tile and any oil present in the ditch must be cleaned up and properly disposed of. I will be following up on location to witness the continuing cleanup. |
