0147720243 2015-01-23 Production Wells No Violations I was on location to follow up to the oil spill that occurred at this location. The flowline has been capped off and the drain tile flushed. New absorbent materials have been installed in the ditch to trap any possible free oil. There is only a small amount of oil present on the pads, but no new oil is present indicating that the leak has been fixed. The ditch will be thoroughly cleaned and remediated when the ice on the water has melted to allow full and complete cleanup. I will be on location to confirm that this cleanup is completed when the ice melts.
0335843074 2015-01-20 Production Wells No Violations I was on location to follow up to the oil spill that occurred on this well 12/15/2014. Following the snowmelt additional oil has flowed from the field tile into the ditch. An excavating crew was brought onto location and began excavating around the wellhead and the flow lines. It was determined that when the well was being worked on the flow line cracked and has been leaking oil that was being pumped by additional wells in the area using a shared flow line. The surrounding wells have been shut in the and drain tile capped to stop the flow of oil. Additional equipment will be brought onto location tomorrow 1/21/2015 to finish the remediation work.
0392060131 2015-05-27 Production Wells No Violations I was on location to follow up the oil spill that occurred at this location on 12/15/2014. Oil is no longer flowing from the drainage tile, after the source of the spill has been corrected. The recent spell of heavy rain storms has caused a strong flushing of the field and drain tile. The 8 pipe that comes from the drainage tile that was broken off from the ground has been fixed. There is no more free oil flowing from the drainage tile at the current time. A boom will be left in the ditch to ensure any possible oil will be caught after being flushed by the forecasted heavy storms moving through the area. At this time the spill has been remediated and the compliance notice has been closed.
1077926072 2014-12-16 Production Wells No Violations I was on location to check the status of cleanup for a spill that occurred into the drain tile near the wellhead and entered a ditch. Sweet Meadow Farm Drainage is on location cleaning up the oil with absorbent pads and booms. The drain tile has been flushed with 1000 gallons of fresh water until it ran clean. All free oil in the ditch and on the water has been removed and disposed of. Two additional booms have been installed in the ditch and absorbent pads placed at the outflow of the drain tile to catch an residual oil that may come out from the upcoming rains. I will follow up on the location tomorrow.

1296323233 2014-12-04 Plug / Plug Back No Violations I was on location to check the status of plugging this well. Petroset bullheaded a 30sk bottom plug through the 4.5 diameter casing. The well pressured up to 1400psi and was shut in for 45min before the valve was opened. The well slightly flowed back, so the plug was shut in for an additional 30min. When the valve was opened again the well was dead and did not flow. The service rig ripped the 4.5 diameter casing at 3700 and pulled up to 3369 to set the Big Lime plug. When the casing was pulled to 3369 the Oriskany Sandstone kicked off and flowed 30bbls of oil. It was decided that the plug would not be set and a swab rig would be brought onto location to try to determine the economic viability of the Oriskany. A permit to plug back will be filed for if it is decided to produce the Oriskany.
1373926738 2015-04-08 Production Wells Violations Noted I was on location to follow up the oil spill that occurred at this location on 12/15/2014. Oil is no longer flowing from the drainage tile, after the source of the spill has been corrected. The recent spell of heavy rain storms has caused a strong flushing of the field and drain tile. 1399574947 2015-01-14 Production Wells No Violations I was on location to follow up to a spill that occurred on 12/15/2014. Oil I still flowing from the drain tile into the ditch. I have contacted C&T Energy and instructed them to immediately get a crew on location to cleanup the free oil and change out the absorbent booms and pads. I will follow up on the spill tomorrow to ensure the cleanup has occurred.
1418743027 2014-12-15 Production Wells Violations Noted A phone call was received from the Stark County Health Department stating that people walking on the walking path near address 13281 Bayton St NE noticed oil coming out of a drain tile pipe into a ditch. I contacted Greg New and Paul Shaw and informed of the spill occurring from this well. The Washington Twn fire department was on location and installed adsorbent pads and booms to control the spill. Ron Fodo of the Ohio EPA was called and informed of the spill. Sweet Meadow Farm Drainage was the contractor called to perform the cleanup. It was determined that when 4.5 diameter casing was ripped to pull it from the well during plugging operations, the Oriskany kicked off and flowed oil. Approximately 5 gallons of oil were not cleaned up and entered the drain tile. After the recent rain the oil flowed to the ditch. An area approximately 30x3 in the ditch was impacted. The banks of ditch were not affected. There is no free oil on the surface or visible near the wellhead. All visible free oil was cleaned up and a boom was installed near the drain tile outflow and at the end of the ditch. A bag of adsorbent pads were put in the ditch to absorb any free oil. In the morning a water truck will be brought onto location to flush the drain tile and remove any oil from the pipe. The oil must be fully flushed from the drain tile and any oil present in the ditch must be cleaned up and properly disposed of. I will be following up on location to witness the continuing cleanup.
1420036261 2014-12-31 Production Wells No Violations I was on location to check the status of cleanup from the oil spill at this location. There is a small amount of oil still flowing from the drain tile pipe and into the drainage ditch. The spill is still being contained by three oil booms and absorbent pads. The location is being check daily by Dan Horning the well pumper and the absorbent pads are being cleaned up and replaced as needed. The spill will be monitored until the oil flow stops.
1422555372 2015-01-29 Production Wells No Violations I was on location to follow up the oil spill that occurred at this location on 12/15/2014. Oil is no longer flowing from the drainage tile, after the source of the spill has been corrected. There is still some free oil locked up in the ice. The operator is going to remove the oil when the ice does thaw. I will follow up to confirm that the oil is removed.

The 8 pipe that comes from the drainage tile has broken off from the ground and a small amount of trapped oil has flowed from the soil. The oil has been contained and cleaned up. There is no more free oil flowing from the drainage tile. A boom will be left to make sure any possible oil will be caught. The final cleanup will occur next week and the compliance notice will be closed at that time.
1444927305 2015-10-14 Final Restoration No Violations I was on location to check the reclamation of this well following the plugging by Phils Well Service. The pits have been closed and the steel plate with the API# has been welded on the casing after it was excavated below grade. The pits and casing will be covered and reclaimed today. The lease road to the well is still present and will need to be excavated and reclaimed before final restoration for the well can be passed. The operator plans to drill another well before the calendar year is over. If a location has been chosen the stone lease road will be reclaimed and used in the construction of the future drilling location.
1701050204 2015-01-21 Production Wells No Violations I was on location to follow up to the oil spill that occurred on this well 12/15/2014. The flowline has been excavated and a cap welded on to stop the flow of oil from the connected wells. The drain tile has been reattached and fixed in the field. A water truck was brought onto location and the field tile has been flushed with 50bbls of fresh water until the outflow ran clear. The outflowing flush water was flowed into a bucket and vacuumed up to the water truck for disposal at an injection well. The absorbent material and booms in the ditch were bagged for disposal and fresh booms and pads were placed. The source of the oil flow has been corrected and the location will be monitored for any new oil that may flow from the field.
1713609733 2017-10-26 Final Restoration No Violations I did a final restoration inspection for this well. The final restoration cannot be passed at this time do to the portion of the lease road in a crop field which has not been removed and the land restored. Also no landowner waiver to leave the lease road has been located at this time. There is no visual signs of this well. The tank battery was shared with wells 151-25540 &25541. I called C & T Energy and left a message for Kathy Lesher (C & T Energy).

2000135042 2015-03-17 Production Wells No Violations I was on location to follow up the oil spill that occurred at this location on 12/15/2014. Oil is no longer flowing from the drainage tile, after the source of the spill has been corrected. The ice in the ditch that was preventing cleanup has thawed and now has flowing water through it. I have contacted C&T Energy and instructed them to have their contractors on location to remove all clean up material on location and any possible free oil still present. I will follow up to ensure the cleanup has occurred and then will close the compliance notice.
