0691380128 2018-02-13 Production Wells No Violations I did a Chiefs Order follow-up inspection for Chiefs Order #2017-112 on this well. The well is in production and the operator is not in compliance with Chiefs Order requiring the suspension of all oil and gas operations in the State of Ohio. The well consist of a pumpjack with an electric motor. The tubing and tubing head flowlines are open. The diked tank battery consists of a steel production tank, a plastic brine tank, a horizontal separator and a meter. The separator was operating at the time of my inspection. The paper chart in the meter is dated 02-12-18.

1313272155 2018-06-04 Production Wells No Violations I did a compliance follow-up inspection for this well. The oil is still coming out of the tiling at the ground level, but it is not in the water going towards an un-named tributary. I spoke with AJ Strawn (Clean-up) who advised me the re-affected area will be cleaned. I will follow-up on the clean-up on 6-11-18

1527595052 2018-05-24 Production Wells Violations Noted I received notifications from Fred Romeo (Emergency Management Coordinator - Oil & Gas) and Larry Antonelli (Ohio EPA) that an oily substance appeared on the landowners property. The landowner initially contacted the Ohio EPA. I moved to 3592 Paris Ave., Louisville, Ohio and met with Landowner Jim Conrad. Mr. Conrad proceeded to show me the areas which are contaminated with oil which follow the drainage path of field tile for approximately 580 ft. between these two coordinates - (40.82433, -81.17742) & (40.82575, -81.17740). The oil appeared after heavy rain events on Tuesday 5-22-18. The oil moved through the field tile north to some unnamed tributary which feeds into Black Run Creek. Booms and absorbent pads were placed to contain any further contamination. I notified Fred Romeo, Larry Antonelli and Stephen Ochs (District Supervisor -Oil & Gas). After further investigation, there was repair work done on the flowline which travels across a tiled field to the horizontal separator in the tank battery. There is oil contamination in the dikes northeast corner and east side of the dike has been repaired. I walked the location I did not located any additional surface contamination outside the dike. I met with Fred Romeo and George Strawn (Representing for North American Petroleum) on location with Mr. Conrad and showed them the extent of the contamination. The contamination is to be cleaned up as discussed in the field. Continue with the containment of the free-flowing water from the drain tile until there is no sign of oil draining from the tile.

The well is in production and not in compliances with Chiefs Order #2017-112 requiring the suspension of all oil and gas operations in the State of Ohio. The well consist of a pumpjack with an electric motor. The valves off the tubing and tubing head are open. The diked tank battery consists of a steel production tank, a plastic brine tank, a horizontal separator and a meter.
1529502046 2018-06-11 Production Wells No Violations I did a compliance notice follow-up inspection for this well. There is water flowing to surface from broken filed tile which has no oil in it. Grass is beginning to grow in the affected soil areas. There is no sign of oil in the unnamed tributary. I spoke with AJ Strawn, he advised me they will continue to monitor the site.
1529580376 2018-06-19 Production Wells No Violations I did a compliance notice follow-up for this well. There is no visual sign of oil on the land or in the unnamed tributary. The vegetation is beginning to grow in remediated area. One section of remediated area had the straw washed away do to a rain event. The collapsed tile section which flows into the unnamed tributary has no visual signs of oil.
1534181362 2018-08-13 Production Wells No Violations I did an Order follow-up for Chiefs Order (2017-112: Failure to Maintain Liability Insurance) and a Compliance follow-up inspection for this well. The well is producing and not in compliance with the Chiefs Order. The well is equipped with a pumpjack. The tank battery consists of a steel production tank, a horizontal separator, a plastic brine tank and a meter. The spill at the tank battery has been cleaned-up. I did not locate any indication of oil purging from the field tile into the crop field or unnamed tributary. The Compliance Notice is passed.
1569846237 2012-01-06 Preliminary Restoration No Violations On 1/6/2012, I performed a ststus check on the above well site. The well is in production, the site is reclaimed and idenitification signs are posted, The Preliminary Restoration is PASSED.
