1616821609 2015-05-06 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The lease road is graveled and entrance is prohibited with a wire line and two poles. The pumpjack well is surrounded by fence with three strand barbed wire. The pumpjack is in good mechanical condition and contains required gauges and valves. The tank battery is shared with wells #25710 and 25828. The tank battery is surrounded by a dike and a fence with three strand barbed wire. The tank battery contains four (4) 210bbl production tanks, two (2) 100bbl brine tanks, poly brine tank, three (3) vertical mechanical separators and a sales line. Vegetation has been established on location and the tank battery, wellhead and road all conform to spacing requirements. At this time preliminary restoration has been passed.
2050557273 2014-01-31 Production Wells No Violations I was on site to perform an inspection on this well in regards to complaint #4589. Complainant states that there is oil in her gas lines and she is afraid that it will get in her water supply. Dominion is on location purging the gas lines and the Health Dept. recommended she call the Division of Oil & Gas because there is a gas well within the vicinity that was drilled a year ago. I talked with Andy Smith, (330-831-1553) C & T Energy Pumper, who stated that the sales line for this well may have sent oil into the Dominion gas line. He has shut down the four wells (other wells #25710, 25770 and 25817 ) that sell into this sales line. I performed an inspection of this well and found a fully fenced in pump jack in good mechanical condition and contained all the proper identification, valves and annular gauges which showed no pressures on any casings but the production casing. The lease road was stable, plowed and the tank battery was fully fenced, contained the proper identification and had a SPPC dike. The tank battery is fully fenced and is shared with well #25710 and 25828 and contained three 100 bbl production tank, two 50 brine tanks and a poly brine tank. It also contained three vertical separators. All the equipment has bull plugs and locks. There are no violations at this time.
