0019806993 2012-10-22 Urban Drill / Deepen / Reopen No Violations I inspected the site construction activities at the Oyster 2 (34151257900000) site on October 22, 2012. Pyramid Pipeline is preparing the site for PEP Drilling Inc., rig number 10. A culvert has been installed in the roadside ditch along Beech Street that would be used as the access point to the site. Silt fencing has been installed along the ditch, parallel to Beech Street and to the west along the tree line to control erosion. A graveled apron is in place and will serve as the access road due to the wellheads close proximity to Beech Street. Pyramid Pipeline is removing the topsoil and stockpiling to the east and west of the wellhead. I called Mike Burkhart from Enervest Operating LLC to have an operator sign installed by the access point with a copy of the drilling permit. Mr. Burkhart called to inform me that an operator sign with a clear tube for the permit will be installed before the end of the day.
1034900863 2012-10-30 Urban Drill / Deepen / Reopen No Violations I inspected the drilling operations at the Oyster 2 (34151257900000) site on October 30, 2012. The drilling contractor for this well is PEP Drilling Inc., rig number 10. The access road consists of a graveled apron from Beech Street approximately 25 feet onto the site that is partially covered in mud due to the rainfall, but the mud is retained on the site. The working pit and blow pit are located to the west of the wellhead; soil berms are being utilized to isolate the pits. The working pit has 3 feet of free board and the blow pit has 3 feet of free board. The pits are cordoned off with safety fencing. The silt fence along the unnamed drainage to the northwest of the wellhead and parallel to Beech Street is intact and preventing migration of sediment from the site. At the time of my inspection, the operator installed the blow out preventer (BOP) and rotating air head and was preparing to test the BOP. The BOP held 500 psi for 15 minutes without a loss of pressure. Herb is the driller on duty. A copy of the permit is located in the dog house and attached to the operator sign at the entrance to the site.
1270586240 2012-10-29 Urban Drill / Deepen / Reopen No Violations I inspected the drilling operations at the Oyster 2 (34151257900000) site on October 29, 2012. The drilling contractor for this well is PEP Drilling Inc., rig number 10. The access road consists of a graveled apron from Beech Street approximately 25 feet onto the site that is partially covered in mud due to the rainfall, but the mud is retained on the site. The working pit and blow pit are located to the west of the wellhead; soil berms are being utilized to isolate the pits. The working pit has 4 feet of free board and the blow pit has 3 feet of free board. The pits are cordoned off with safety fencing. The silt fence along the unnamed drainage to the northwest of the wellhead and parallel to Beech Street is intact and preventing migration of sediment from the site. At the time of my inspection, the operator drilled an 11 inch borehole to a depth of 904 feet and was setting 8.625 inch casing and cementing to surface. The blow out preventer and air head were not in place, but the operator did use an air body attached to the 11.75 inch conductor casing to drill on air and fresh water. Jeff is the driller on duty. A copy of the permit is located in the dog house and attached to the operator sign at the entrance to the site.
1352299138 2012-10-26 Urban Drill / Deepen / Reopen No Violations I inspected the Oyster 2 (34151257900000) site on October 26, 2012. PEP Drilling was setting up rig number 10 and should spud the well early Saturday morning October 27, 2012. Pyramid Pipeline was on site finishing the pits and safety fencing was installed. The operator sign was posted at the access road and included a copy of the permit. The access road consists of a graveled apron from Beech Street approximately 25 feet onto the site. The pits are located to the west of the wellhead. Silt fence has been installed along the unnamed drainage to the northwest of the wellhead and parallel to Beech Street.
1352487064 2012-11-01 Urban Drill / Deepen / Reopen No Violations I inspected the drilling operations at the Oyster 2 (34151257900000) site on November 1, 2012. The drilling contractor for this well is PEP Drilling Inc., rig number 10. The access road consists of a graveled apron from Beech Street approximately 25 feet onto the site that is partially covered in mud due to the rainfall, but the mud is retained on the site. The working pit and blow pit are located to the west of the wellhead; soil berms are being utilized to isolate the pits. The working pit has 3 feet of free board and the blow pit has 3 feet of free board. The pits are cordoned off with safety fencing. The silt fence along the unnamed drainage to the northwest of the wellhead and parallel to Beech Street is intact and preventing migration of sediment from the site. At the time of my inspection, PEP was drilling a 7.875 inch borehole and the current depth was 3465 feet. The blow out preventer and rotating air head are in place and PEP is drilling on air with dust suppression. Herb is the driller on duty. A copy of the permit is located in the dog house and attached to the operator sign at the entrance to the site. The top of the Big Lime was recorded at 3098 feet and the Oriskany at 3365 feet.
1530082212 2014-01-14 Production Wells No Violations I was on location to inspect the Oyster 2 well site. My inspection found the access road is in good condition and graveled to the tank battery and the wellhead. A gas powered pumping unit is installed on the wellhead and was running at the time of my inspection and is surrounded by a barbed wire fence. The tank battery is surrounded by a SPCC dike and consists of one 210 bbl stock tank, one 2100 gallon ploy brine tank, and one separator. The proper operator signs are attached to the tank battery. Preliminary restoration can not be passed at this time for this well.
1977625497 2012-12-03 UrbanPreliminary Restoration No Violations I inspected the Oyster 2 (34151257900000) well site on December 3, 2012. My inspection found the access road is in good condition and graveled to the tank battery but not to the wellhead. A gas powered pumping unit is installed on the wellhead and was running at the time of my inspection. The site has been rough graded. The pits have been solidified and filled. The tank battery consists of one 210 bbl stock tank, one 2100 gallon ploy brine tank, and one separator. The proper operator signs are attached to the tank battery. Preliminary restoration is due by April of 2013.

The drilling contractor for this well is PEP Drilling Inc., rig number 10. The access road consists of a graveled apron from Beech Street approximately 25 feet onto the site that is partially covered in mud due to the rainfall. The working pit and blow pit are located to the west of the wellhead; soil berms are being utilized to isolate the pits. The working pit has 5 feet of free board and the blow pit has 9 feet of free board. The pits are cordoned off with safety fencing. The silt fence along the unnamed drainage to the northwest of the wellhead and parallel to Beech Street is intact and preventing migration of sediment from the site. At the time of my inspection, the operator had drilled a 17.5 inch borehole to a depth of 197 feet and was setting 11.75 inch conductor casing and cementing to surface. The blow out preventer and air head were not in place; the operator is set up to drill on fluid. Herb is the driller on duty. A copy of the permit is located in the dog house and attached to the operator sign at the entrance to the site. The Big Injun Formation was recorded at 176 feet.
