0128627469 2012-05-22 Drill / Deepen / Reopen No Violations I conducted an inspection of the drill site while on location to witness the drilling of the conductor hole. Ranson Drilling was using an auger style rig to drill a 24 conductor hole. The cellar was in place, lined with tin horn. The entire location was graveled and fenced with chain link. An operators sign and drilling permit was posted at the lease road entrance. No violations at this time
0545184234 2012-06-01 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nomac Rig #14 was not drilling. The rig was undergoing repairs to the top-drive unit. All of the drilling equipment was on lined containment barriers. The site was fenced and a security guard was posted at the entrance. The permit was displayed in the trailer and an operators sign was posted at the site entrance. No violations at this time.
0570475013 2012-06-20 Plug / Plug Back No Violations On the day of my inspection I found that Mercer Well Service had successfully fished drill pipe, drill collars and bit out of the hole. They were in the process of running 2-7/8 tubing in to a depth of 530. Plugging will commence at 07:00 on 6/21/12. No violations.

PSI was taking split-spoon core samples at cardinal directions from the well bore. The sample to the south was cored to a depth of 30, the sample to the east was cored to a depth of 50, the north and west samples will be cored to a depth of 20.
0843960900 2018-12-21 Final Restoration No Violations I met with Lucas Shipper (Encino) on location to check the well location for the existence of an unplugged conductor casing. No unplugged conductor casing was located. The final restoration for this well cannot be passed at this time. A Landowner Waiver (Form 5) is pending for this well. Once Landowner Waiver has been received by the Uniontown Office of Oil & Gas, a Final Restoration Inspection can be completed.

1328358968 2019-04-29 Final Restoration No Violations My inspection found no visual evidence of the well. The tank battery was never constructed. There is no active erosion. A Landowner waiver was received by the Uniontown Office of Oil & Gas on 04-03-2019. The Landowner waiver waives the following: The backfilling and grading of the wellsite - The entire pad is to remain, The backfilling and grading of the access road - The entire lease road will remain and the fencing is to remain. The Final Restoration is passed.

The #3 well was a lost hole and has been plugged. The well has been cut down below grade. There is no equipment present on the location.
