The access road is in good condition and graveled to the well site. The silt fence along the unnamed drainage to the west and south of the site is intact and in good condition. Upon arriving at the site, Enervest Operating LLC Drilling Superintendent, Jeff Foley, had opened the valve on the 8.625 inch annular space due to over pressurization in the open completion. The well is open hole from approximately the Utica Formation to the base of the 8.625 inch surface casing. Fluid containing drilling mud was bubbling out of the open valve and draining into the working pit. Enervest Operating LLC planned to leave the valve open and plumb the port to a ploy tank for containment. |

My inspection found the access road is in good condition and graveled to the well site. The silt fence along the unnamed drainage to the west and south of the site is intact and in good condition. The well has an electric pumping unit installed and the pumping unit was running at the time of my inspection. The site has not been reclaimed, preliminary restoratio is due by April 2013. The pits have been solidified and filled. The annulus for the 8.625 inch casing has been plumbed into the flow line and the pressure gauge shows 40 psi. The pressure gauges for the 4.5 inch casing and tubing are 1750 psi and 240 psi respectively. The tank battery consists of one 210 bbl stock tank, one 2100 gallon ploy brine tank, and one separator. An additional 210 bbl stock tank is on site to be installed due to the high volume of oil being produced. Bull plugs are installed on all flow lines. A spark arrester is installed on the vent stack of the 210 bbl stock tank. The proper operator signs are attached to the tank battery and pumping unit. |

PEP Drilling drilled an 11 hole to a total depth of 768 at the kelly bushing (KB +10). PEP set 758 (KB) of 8.625 surface casing in the hole and Universal rigged up and cemented the casing with 270 sacks of Type 1 cement. Circulation was never established to the top of the annulus and cement returns were never achieved. I informed PEP and Enervest that we would need to wait 8 hours prior to breaking off and that a cement bond log would need to be run to evaluate the top of the cement. |

Appalachian Well Surveys rigged up and ran a cement bond log on the 8.625 casing in the hole. It was determined that the top of the cement was at 158 from the top of the casing (+12). Enervest decided to run 1 tubing down the annulus to cement up the back side of the casing. PEP Drilling ran 5 joints of 1 tubing in the hole to 126 at ground level and the last joint was not run due to an obstruction. Universal Well Services rigged up and pumped 100 sacks of Type 1 cement with good returns to the surface. The cement fell back 5-6 in the annulus. |
