0071651266 2018-11-21 Preliminary Restoration No Violations My inspection found the pits have been filled and the land restored around the well and tank battery locations. There are no signs of active erosion at either location. The preliminary restoration is compliant with Ohio Revised Code 1509.072(A). The preliminary restoration is passed. The well is equipped with a pumpjack. The tank battery consists of four steel production tanks, three plastic brine tank, four vertical separator and four meters. The tank battery is shared with wells #151-25807, 25771 and 25858. The well is legibly identified. No compliance issues are cited.
1240155514 2012-09-25 Drill / Deepen / Reopen No Violations Kilbarger #17 rig drilling 7.875 borehole. Current depth is 3,080. Pits are in good working condition and dust suppression is sufficient. Grading site to remove mud.
1348367386 2012-09-22 Drill / Deepen / Reopen No Violations On site for the cementing of surface casing. The 11 borehole was drilled to a depth of 942. Circulation was not established during drilling. 922 of 8.625 casing was set using 400 sacks of cement (100% excess). Universal Well Services ran 60 bbls of fresh water before 30 bbls of gel and flake. Cement had 3/4 pound of flake added. No cement returns. Will run cement bond log tomorrow morning.
1348425094 2012-09-23 Drill / Deepen / Reopen No Violations Cement was not circulated to surface while setting the surface casing string. Appalachian Well Surveys ran cement bond log and found top of cement was at 196 KB. Top of cement inside the 8.625 casing tagged at 890. Kilbarger drilling crew inserted 189 of one inch line down the back side for grouting. Universal Well Services established circulation and grouted the surface casing annular space using 100 sacks of cement. Good cement returns to surface but top of cement fall back. Kilbarger will tag top of cement after 4 hours and top off as needed.
1508909735 2012-09-21 Drill / Deepen / Reopen No Violations I arrived on site to inspect the cementing of conductor casing. The conductor hole was drilled to a total depth of 191 using a 17.5 bit. A total of 179 of 11.75 casing was set (KB). Circulation was established and 300 sacks of cement were pumped downhole with 24 bbl of returns.
2139327151 2012-11-20 Preliminary Restoration No Violations I inspected the Mason 4 (34151258060000) well site on November 20, 2012. My inspection found the access road is in good condition and graveled to the well site. The site has been rough graded and preliminary restoration is due by April 2013. The pits have been solidified and filled.
