My inspection found Universal Well Services was contracted by Enervest Operating LLC to complete the hydraulic fracturing of the well. The job started at 09:30 and ended at 10:30 with an average operating pressure of 2536 psi. The average slurry rate was 15 bbls of water per minute. A total of 25,276 gallons of water was used to complete the operation.

The drilling contractor for this well is Advent Drilling. My inspection found an operator sign at the entrance to the site. The access road is graveled from Deerford Avenue to the well site. Silt fencing has been installed along the northern and southern perimeter of the well site, parallel to the unnamed tributary to the south, and on the back side of the soil berms. The working pit and blow pit are located to the south of the wellhead and are cordoned off with safety fencing. The working pit has 6 feet of free board and the blow pit is empty. Advent drilled a 14.75 inch borehole to a depth of 453 feet and was setting 11.75 inch conductor casing and cementing to surface. The blow out preventer and rotating air head were not in place; Advent is set up to drill on fluid. Jeff is the driller on duty. A copy of the permit is located in the dog house.

I met with Mike Burkhart, Enervest Operating LLC, to discuss the location of pits and installation of silt fencing. My inspection found an operator sign at the entrance to the site. Energy Services Corporation (ESC) was completing the installation of the access road to the wellhead. Straw bales are installed adjacent to the culvert along Deerford Avenue to prevent erosion. Advent Drilling was delivering equipment. Soil berms are utilized to mitigate erosion and off site sedimentation along the lease road. Silt fencing will be installed along the north, east, and west perimeter of the well site.

Energy Services Corporation (ESC) dug and lined the blow pit and will dig the working pit after Advent Drilling has delivered all equipment. The blow pit is cordoned off with safety fencing.

1989262740 2014-03-10 Preliminary Restoration No Violations During my inspection I found identification for the well at the lease road entrance and the tank battery. The tank battery contains two 210 bbl production tanks, a 50 bbl poly brine tank and a horizontal separator. The pump jack well is run by natural gas and has a chemical container contained within a dike near the well. The wellhead has all necessary gauges and bull plugs. Vegetation has been established at the location. GPS coordinates were updated due to wrongly plotted previous points. Preliminary restoration is passed.
