0394227672 2012-11-19 Urban Drill / Deepen / Reopen No Violations I inspected the Shahan 1D (34151258170000) site on November 19, 2012. The drilling contractor for this well is Advent Drilling. My inspection showed the access road was graveled to the well site and in good condition. The access road does not show signs of erosion. The operator sign is at the access road with a copy of the permit attached. The working pit and blow pit are located to the east of the wellhead. Both pits have 4 feet of free board. The pits were cordoned off with safety fencing. At the time of my inspection, Advent had set 4592 feet of new 4.5 inch casing and Petroset Cementing Services Inc. was on site to cement the casing. The cellar is lined. The silt fencing along the eastern and northern perimeter of the site is in good condition and preventing sediment migration into the wetland area northeast of the wellhead. Jeff is the driller on duty. A copy of the permit is located in the dog house.
1272143599 2012-11-13 Urban Drill / Deepen / Reopen No Violations I inspected the Shahan 1D (34151258170000) site on November 13, 2012. My inspection showed the access road was graveled to the well site and in good condition. The access road does not show signs of erosion. The operator sign is at the access road with a copy of the permit attached. The drilling contractor, Advent Drilling, tested the BOP and cement for the 8.625 inch casing at 500 psi for 20 minutes with no pressure loss. The BOP and rotating air head were in place and chained to the substructure so the drilling contractor could drill on air. The working pit had 3 feet of free board and the blow pit had 8 feet of free board. The pits did not have safety fencing installed and I called Charlie Keeney to discuss the requirement of fencing in an urban location. By 2:30 pm I received a message from Rob Frock informing me that the safety fencing was installed to isolate the pits and the operator started drilling the 7.875 inch borehole on air. A 500 bbl water tank and topsoil were staged in the northeast corner of the site to mitigate noise to the residents north of the location.
1302270618 2012-11-09 Urban Drill / Deepen / Reopen No Violations I inspected the site construction activities at the Shahan 1D (34151258170000) site on November 9, 2012. My inspection found Joe Little Excavating preparing the site for Advent Drilling. An operator sign with a copy of the permit attached is located at the entrance to the site from State Route 619. The access road is graveled to the well site. Silt fencing has been installed along the eastern and northern perimeter of the site to prevent sediment migration into the wetland area northeast of the wellhead. The working pit and blow pit are located to the east of the wellhead. Topsoil has been stockpiled to the north and west of the wellhead. The drilling contractor was setting up the drilling rig at the time of my inspection and should spud the well tonight.
1356610941 2012-12-11 UrbanPreliminary Restoration No Violations I inspected the Shahan 1D (34151258170000) well site on December 11, 2012. My inspection found the access road is in good condition and graveled. Joe Little Excavating was installing a pumping unit on the well. The silt fencing along the eastern and northern perimeter of the site is in good condition and preventing sediment migration into the wetland area northeast of the wellhead. The pits have been solidified and filled. The tank battery consists of one 210 bbl stock tank, one 100 bbl steel brine tank, and one mechanical separator. The proper operator signs are attached to the tank battery. Construction at the site is not complete, the tank battery does not have fencing and landscaping installed yet. Preliminary restoration has been extended until weather permits and erosion controls will remain in place unitl preliminary restoration is complete.
1363907410 2013-04-16 Production Wells No Violations The Uniontown field office received two phone calls pertaining to odors and noises coming from the site. Both callers expressed concerns about what was happening at the site. I went to the site in the afternoon to check on what activities were taking place. Upon my arrival at approximately 3:25pm, I found a service rig on the site, but no personnel. The rig appeared to be in the process of conducting a workover. The 2 production tubing and rods had been removed from the well and were racked. It appeared that the rig was in the process of cleaning paraffin from the rods and tubing. A valve on the well was open to the flow line to the tank battery. The following morning (04/17/2013) I called inspector Bob Roush regarding following up on the workover with the company. I called both people who had called into the office, and told them of my findings that a workover was underway, and that the noises and odors were normal, and would be gone upon completion of the workover.
1384445012 2013-11-12 Production Wells No Violations I checked the wellsite in reference to complaint #4534. The well is fenced and newly painted with a pumpjack. There are no signs of any oil leaks or issues with the well. There is a topsoil stockpile on location which could have been seen as an oil spill. There are no violations for this well and this complaint can be closed at this time. Inspector Roush has had the same findings and has inspected this well in the past regarding similar issues.
1606897873 2014-01-31 Production Wells No Violations I was on site to perform an inspection on this well in regards to complaint #4589 Complainant states that there is oil in her gas lines and she is afraid that it will get in her water supply. Dominion is on location purging the gas lines and the Health Dept. recommended she call the Division of Oil & Gas because there is a gas well within the vicinity that was drilled a year ago. I talked with Andy Smith, (330-831-1553) C & T Energy Pumper, who stated that the sales line for this well may have sent oil into the Dominion gas line. He has shut down the four wells (other wells #25710, 25770 and 25828) that sell into this sales line. I performed an inspection of this well and found a fully fenced in pump jack in good mechanical condition and contained all the proper identification, valves and annular gauges which showed no pressures on any casings but the production casing. The lease road was stable, plowed and the tank battery was fully fenced, contained the proper identification and had a SPPC dike. The tank battery contained a 100 bbl production tank, 50 bbl brine tank and a vertical separator. All equipment contained bull plugs and locks. There are no violations at this time.
1942853116 2012-11-15 Urban Drill / Deepen / Reopen No Violations I inspected the Shahan 1D (34151258170000) site on November 15, 2012. The drilling contractor for this well is Advent Drilling. My inspection showed the access road was graveled to the well site and in good condition. The access road does not show signs of erosion. The operator sign is at the access road with a copy of the permit attached. The working pit and blow pit are located to the east of the wellhead. Both pits have 6 feet of free board. The pits were cordoned off with safety fencing. Advent is drilling a 7.875 inch borehole on air and soaping. The current depth is 2728 feet. The blow out preventer and rotating air head are in place and chained to the substructure. The cellar is lined. The silt fencing along the eastern and northern perimeter of the site is in good condition and preventing sediment migration into the wetland area northeast of the wellhead. Jeff is the driller on duty. A copy of the permit is located in the dog house. The top of the Big Lime was recorded at 2646 feet.
