1368707086 2013-05-15 Casing String Witnessed No Violations I was on location to witness the cementing operations of the Surface string. The 17.5 borehole was drilled to a total depth of 475 KB @ 1600 Hrs. Circulated and swept hole with fresh mud pill. Pull out of hole and stood back the drill collars. Ran 11 joints of 11.75, 32 Lbs./ft., R-3, Limited service, ST&C casing to 463 KB. Circulate on the casing. Moved in Universal Well Service ( UWS ) and rigged up to cement. UWS broke circulation, pumped 15 Bbl. Gel ( 300 Lbs. ) with .25 lbs./sack cello-flake ( flake) and cemented with 550 sacks of Class A cement with 3% CaCl and .25 lbs./sack flake. Slurry was displaced with 54 Bbl. Fresh water to 443 KB. Plug down at 2240 Hrs. Cement circulated with 23 Bbls. of cement returns pumped to the pit.
1369149253 2013-05-17 Casing String Witnessed No Violations I was on location to witness the running and cementing of the 8.625 casing. PEP Rig 10 drilled an 11 hole to 1035 KB total depth. The Rig ran 24 joints of 8.625 23 Lbs./ft, A-500, ST&C casing ( 2900 psi- burst, 1213 psi- collapse ) to 1025 KB. Centralizers were ran on the 1 , 4 , 7 , 10 , 13 , 16 , 20 th joints. Universal Well Services cemented with 325 sacks Class A cement, 3% CaCl, .25 Lbs. / sack cello flake after pumping a 15 Bbl. Gel flush. The tempeture of the mix water was 56 and pH was 6. Cement returns were seen at the surface with 20 BbLs. circulated to the working pit. Ran casing baffle to land the plug. Left 40 of cement in pipe. No violations were observed.
1607027978 2014-08-20 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The lease road is graveled and leads up to a pump jack well in good mechanical condition and contains all necessary gauges. The tank is surrounded by a dike and has proper I.D. signage. The tank battery contains two 210bbl production tanks, a 100bbl brine tank and a horizontal separator. Vegetation has been established and there are no erosion control issues. At this time preliminary restoration has been passed.
