-1983240146 2013-10-24 0:00 CS No Violations
1382648321 2013-10-24 Casing String Witnessed No Violations I was on location to observe the cementing of the 8.625 casing through the Big Injun . Poulson Drilling ( P D C ) drilled an 11 hole to 597. PDC ran 13 joints of 8.625, 23 Lbs., AT-50,R-3, ERW, ST&C casing. Casing was centralized with Bow Spring centralizers on collars 1, 4, 7, 10. Universal Well Services conditioned the wellbore with a 20 Bbl. Gel preflush followed by 180 sacks of Class A , 3% gel, 1/4 lbs. per sack Celloflake cement slurry. The cement slurry was circulated to surface and into the working pit. I observed 5 Bbl of cement returns. Water temp. was 49 degrees with the pH -7. I requested a wait on cement ( WOC ) time of 8 hrs. No violations were observed.
1390580962 2014-01-24 Production Wells No Violations I was on location to perform a status check of this well. The lease road is graveled and in good condition. The tank battery contains a 210 barrel production tank, 50 barrel brine tank and a horizontal separator. The tank battery has a SPCC dike, proper signage. The pumpjack well has annulus pressure gauges, signage and is electric operated. The location still has equipment on location to restore the location when the weather improves. I will follow-up again to pass preliminary restoration. There are no violations at this time.
1418839820 2014-12-17 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The lease road is gravel and leads up to a pumpjack well with proper ID, gauges and valves. The tank battery contains a 210bbl production tank, poly brine tank, horizontal separator, ID sign and is contained within a dike. Vegetation has been established and preliminary restoration has been passed.
1577494056 2013-10-28 Casing String Witnessed No Violations The well has been drilled to 5454 and 4.5 10.5lb/ft casing has been run and circulated. Universal rigged up and pumped the cement job as planned. The cement head would not open so a 4.5 swedge had to be used to cement the casing following the well being circulated with the rig mud pump. Due to the well staying static while trying to fit the cementing head a small gas pocket came up through the annulus but was depleted before the plug was dropped. 34.5 barrels of class A cement was pumped and a total of 89 barrels of displacement was used .
1983240146 2013-10-24 Casing String Witnessed No Violations I was on location to observe the pressure testing of the Blow Out Preventor ( BOP ). Poulson Drilling Corp. ( PDC ) ran the drill string back into the 8.625 casing. An Annular BOP was attached to the top of the casing string. The BOP was closed on 3.5 drill pipe and pressured up to 350 psi. I noticed that the fittings were leaking and cancelled the test until repairs could be made. PDC again pressured up to 370 psi and held the pressure for 20 minutes without any leakage. The Annular BOP was released and flowed out the Blowline into the pit. I passed the BOP @ 0132Hrs. No violations were observed.
2116183063 2013-10-22 Casing String Witnessed No Violations I was on location to observe the running and cementing of the Conductor casing string. Poulson Drilling Corp. ( PDC ) drilled the 14.75 conductor hole to 190 G.L. Glacial drift was found from 0 to 147 ( samples ) and comprised of pea gravel. PDC ran 4 joints of 11.75, 32 #, R-3, ERW, AT-50, ST&C casing to 168.30 set at G.L. The casing string was scheduled for 2 centralizers, but due hole conditions only one was used. Universal Well Services ( UWS ) broke circulation with 20 Bbl of gel and followed with a cement slurry of 150 sacks of Type 1, 3% gel, 1/4 # per sack celloflake. UWS displaced cement slurry with 18.2 Bbl of fresh water. I observed 26 Bbl of cement returns flow back to surface and into the pit. Twenty feet of cement was left in the casing by request. No violations were observed at this time.
