0143302266 2014-02-12 Completion Testing No Violations My status check inspection found two J Hawk trucks on location circulating the water currently in the frac tanks on location. They are planning on fracking the well tomorrow and the tanks are started to freeze. The circulation will stop the freezing and allow them to use the water for the frac job.
0225452551 2014-02-07 Completion Testing No Violations

-displace the acid with 10 barrels of water and let it sit over the weekend before coming back Monday. They plan to attempt to frac through the 4.5 casing so that they do not have friction losses due to the restrictive diameter of the 2 7/8 tubing.

0813957079 2013-12-10 Urban Drill / Deepen / Reopen No Violations I was on location to check the status of Poulson drilling this urban well. The rig is currently drilling at 4700 and will reach TD at 5171. There has been no gas shows on the well and I have checked the gas meter and confirmed this. They are drilling the well on air and will be running used 4.5 10.5# casing which has been tested to 4400. There are no violations at this time.
0952141965 2014-02-13 Completion Testing No Violations I arrived on location after the frac crews left the location. Bishop Wells Services informed me that they were able to achieve breakdown and treat the well at 25 bpm. Bishop was currently flowing back the well but it is going very slow due the gel being thick. They expect the flow back to take longer then usual due to the weather.
1386027162 2013-12-02 Administrative Inspection No Violations I was contacted by Enervest regarding the pits for this well. While digging them they hit sand and gravel and wanted them to be moved to another location. Inspector Roush was on location and approved the movement of the pits to a location with a clay bottom. They will be monitored and checked again before drilling begins.
1389897217 2014-01-13 Drill / Deepen / Reopen No Violations I was on location to witness the setting of a packer in this well for the stimulation and formation breakdown. The Clinton was not able to be broken down with the pump trucks so 2 3/8 J-55 tubing is going to be set in the well with a packer to allow up to 6500psi of pressure to be applied to the well for breakdown. The packer was set at 4800 which is below cement top. 500 gallons of 28% acid was set in the well to achieve breakdown. There are no violations at this time.
1430846082 2015-05-05 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The electric driven pumpjack well is not equipped with the required annular pop off and pressure gauges. I have contacted Enervest and requested one be installed before preliminary restoration be passed for the well. I will follow up and check for full preliminary restoration when the required pop off and pressure gauges are installed.
143302266 2014-2-12 0:00: CT No Violations 1.00
1537936511 2013-12-04 Urban Drill / Deepen / Reopen No Violations I was on location to check the status of Poulson drilling. The rig is currently drilling at 189 and will drill to around 550 where they will then set surface casing. They expect to be cementing tomorrow mourning. The rig is drilling the hole on fluid at this time.
1548263643 2013-12-03 Drill / Deepen / Reopen No Violations I was on location to check the status on Poulson drilling. The rig is currently reaming the 20 hole to 120 where they will set 16 conductor through the drift that was encountered on this well. This was a contingency string for this well.
