0200901833 2014-06-11 Production Wells No Violations I was on location to check the status of this well being put into production. The flow lines for the pump jack has been installed and buried. The pump jack is currently pumping and the well is now in production. Flow back equipment and some lighting are still present on location and will be moved off soon. There are no violations at this time.
0479668263 2014-03-04 Drill / Deepen / Reopen No Violations I was on location to witness the testing of the blowout preventer (BOP). The weather delayed this process more then 24 hours and the continual freezing of the flow lines and stack limited their testing. They were thawing the stack and then tested the BOP to 1000 psi and held it for 15 min. The test passed. They are going to continue to thaw the stack and hammer drill their 7 7/8 vertical section to their kickoff point of 4600. Once they reach this depth they will do a complete BOP test before they begin to build their curve and enter the lateral of this well. There are no violations at this time.
0571600914 2014-04-29 Casing String Witnessed No Violations I was on location to witness the running of a cement bond log by Appalachian Well Services on this well. Cement was found from 4612 to 4482. This is less than the 1000 required for a horizontal well. The limited cement fill-up was due to a large washout present in the Salina section of the Big Lime. The washout limited the vertical extent of the cement. A remedial squeeze job was required in order to fill the annular space to the required level. Four perforations with 60 degree phasing were shot at 4386 in the 5.5 casing. This depth was chosen because there was a clean top of cement at 4418, but a casing collar was located near that depth. The collar was to be avoided during perforation. A total of 360sks of Unifill cement were pumped into the well and were followed up by 50sks of class A 2% CaCl and gel. A cement bond log will be run again tomorrow to assess the effectiveness of the squeeze job.
0631591206 2014-02-12 Drill / Deepen / Reopen No Violations I inspected this location on 2/21/2014. Highlands Drlg was still rigging up. I asked the Operator to install some silt fencing and hay bales to abate possible off- location drainage. The Operator complied and installed the requested materials. No violations were identified during this inspection.
0734076649 2014-05-27 Completion Testing No Violations I was on location to check the status of the flow back for this well. They are currently flowing back the well at 10-15 bbl/hr. The production equipment for this well has been ordered and the well will have rods installed in the near future to ready it for production. The tank battery has been installed and consists of 2 210 bbl brine tanks and 2 210 bbl production tanks. The battery also consists of a vertical separator and has a dike surrounding the battery. There are no violations at this time.
0759218465 2014-03-02 Casing String Witnessed No Violations I was on location to observe to the placement and cementing of the intermediate string of casing in the Rummel Farms # 1-H. Highlands Drlg. Rig # 7 drilled a 11 hole, on air, to 1521 KB. The rig then placed 1500 of 8.625 24 lbs. / ft., J - 55, ERW, R-3 casing in the wellbore. Casing hardware included a guide shoe, landing baffle ( 1457 KB ), and 10 centralizers. Universal Well Services ( UWS) connected to the casing , mixed, displaced, and circulated a cement slurry ( 450 sacks of Type 1, 3% CaCl2, .25 lbs. / sack celloflake ) back to surface ( 22 Bbls. of cement returns ). Water temp. was 56, pH- 7. Plug down @ 2337hrs.
0813131693 2014-02-21 Casing String Witnessed No Violations I was on this location to observe the cementing of the Conductor string of casing. Highlands Drlg. drilled a 22 hole to 93 K.B.. A Conductor string consisting of 2 joints of 16 , 75#, H-40, R-3, ERW, ST&C casing was ran to a depth of 81 K.B. . The last 20 had to be circulated down due to fill-up in the wellbore. A centralizer was placed at 41 K.B. . Universal Well Services broke circulation and mixed a slurry of 160 sacks of Type 1 cement , blended with 3 % CaCl2, .25 # / sack celloflake. Water temp -38, pH-7. I observed approximately 5 Bbl. Of cement returns back to surface. I did not identify any violations during this inspection. Glacial drift was 0 to 28, Coal- 30 - 33.
1010415965 2014-02-23 Casing String Witnessed No Violations I was on location to witness the setting and cementing of the 16 Conductor casing. Drilling contractor was Highlands Drilling Rig # 7, who drilled a 22 diameter hole to 93 , as measured from the Kelly Bushing ( KB ) . The Glacial drift was 29 thick with a 2 Coal seam at 30. After reaching total depth and circulating on the wellbore for 1 hr., the drillstring was removed. Two joints of 16 casing were ran in the wellbore, but hit an obstruction at 71. Universal Well Service ( UWS ) hooked up to the 16 casing and circulated it down to 81. Total tally of casing was 81 , which consisted of two joints of 16, 75 lbs./ft, H-40 grade casing. A centralizer was placed at 42 KB. UWS established circulation and cemented the casing by circulating the cement slurry to surface. UWS used 160 sacks of Type 1, 3 % CaCl2, .25 lbs. / sack celloflake. I weighed the cement slurry and found it weighed 15.3 lbs. / gal. Water temp. was 37 degrees and pH was 7. A wait on cement time of 9 hours was prescribed due to the low mix water temp. and bottom hole temp. I observed 5 Bbls. of cement slurry returned back surface. I did not identify any violations during this inspection.
1163353899 2014-05-01 Drill / Deepen / Reopen No Violations I was on location to witness the running of a cement bond log by Appalachian Well Services on this well. Cement was found from 4386 to 2850. This now gives them the required cement coverage for this well. They are now currently rigging up and drilling out the cement with a 4.75 mill tooth tri-chome bit. After the drill out they are going to pressure test the casing and perforation holes to at least 3000psi. Stallion frac tanks are being brought on location to prepare for the frac operations to begin May 7th. There are no violations at this time.
1171129632 2014-03-20 Drill / Deepen / Reopen No Violations During my inspection I found the Highlands #7 rig to be drilling the 7-7/8 curve with a Baker Tricone at 5,463. They are currently in the white clinton and are going to pull out of the hole and use a 6-3/4 Baker I-Rev bit to drill the lateral of the well. There are no violations.
1192287400 2014-02-24 Drill / Deepen / Reopen No Violations I inspected this location on 2/24/2014. I found that Highlands Drilling was drilling a 14.75 hole at 176 KB.. Upon my inspection of the compressors and boosters area I found that a .5 inch ball valve had failed on a fuel volume tank and released diesel fuel on the ground. The affected area was 5 by 7. Since the ground was frozen , I estimated the absorption was minimal. I immediately contacted Todd Tetrick ( Enervests VP of Drilling ) with whom I re-inspected the area. Highlands Drlg. workers were using oil absorbent pads to collect the free fuel. I told Mr.Tetrick that the affected area will have to be excavated down to a minimum of 18, remove the soil and replace with clean soil. A sealed roll off container supplied by Cowboy Industries was filled with the excavated soil. I requested a disposal manifest from Enervest. New topsoil was brought to replace the removed soil.
1365422870 2014-02-26 Casing String Witnessed No Violations I was on location to witness the cementing of the 11.75 Surface casing. Highlands Drlg. Rig #7 drilled a 14.75 diameter hole to 740 KB. on fresh water drilling mud. The Big Injun sandstone was found from 490 to 620 KB. A casing string comprising of 17 joints of 14.75, 42 lbs. / ft. , J-55 , casing was installed to 724. The casing hardware consisted of a guide shoe, a baffle ( @ 682 KB ) and 5 centralizers ( on collars 1,4,8,12,15 ). Universal Well Services ( UWS) connected to the casing and established circulation , mixed a cement slurry ( 410 sacks of Type 1 , 3% CaCl2, .25 lbs. / sacks ) , displaced the cement slurry back to surface with a wiper plug landed at 682KB. Water temp. was 42, pH -7. I weighed the cement at 15.4 Lbs. / gal. Eleven barrels of cement slurry was observed returned to surface. I did not observe any violations.
1391806680 2014-02-07 Drill / Deepen / Reopen No Violations I was onsite to check the start of construction for this location. The location is starting from the road at this time with the drainage culverts being installed at the beginning of the lease road. They expect to start drilling this well estimated within a month.

A culvert has been installed at the road and the lease road is partially graded and gravel is being leveled out. The pad is partially excavated and grading has began. There are no violations at this time.
1393173213 2014-02-20 Drill / Deepen / Reopen No Violations I was on location to observe the moving in and rigging up ( MIRU ) of Highlands Drilling Rig #6 and to inspect the lining of the pits. I noticed water had gathered in the blow pit so I instructed the Operator to pump that water off prior to lining the pit. The Operator complied. A 20 mil liner was used. No violations were indentified at this inspection.
1393940586 2014-02-22 Drill / Deepen / Reopen No Violations I was on location to observe the mobilization of Highland Drilling Rig # 7 to spud the Rummel Farms # 1-H. The pits were lined with a 20 mil liner and liquid tight. The cellar was lined with 20 mil pit liner and directed all drilling fluids through 12 plastic pipes to the pits. I inspected the compressors, booster and mud pump area and found ( although not running at this time ) to be leak free. The well was spudded at 1100hrs. I did not identify any violations at this time.
1393956478 2014-03-03 Drill / Deepen / Reopen No Violations I was on location to check the status of the of Highland #7 drilling rig. The night before they set there intermediate casing to 1500. They were expecting to be testing the blow out preventer (BOP) today but cold weather has hindered the operation. They were nippling up to set the BOP and were expecting to test it later tonight and begin dust drilling their 7 7/8 hole to their kickoff point at midnight. There are no violations at this time.
1394213054 2014-03-07 Drill / Deepen / Reopen No Violations My inspection found that the rig was currently about to pick up a new bottom hole assembly with a 7 7/8 trichome bit. The Highland rig was fishing the air hammer sub from the hole. It is believed the Oriskany caused excessive wear the on the air hammer. They chose the trichome bit to have a reliable drilling schedule through the salt formation until they reach their kickoff point and test the blow out preventer before they begin to build their curve. There are no violations at this time.
1396464935 2014-04-02 Drill / Deepen / Reopen No Violations During my inspection I found the Highlands #7 rig to be currently rotate drilling the lateral of the well at 6276. Their current rate of penetration is 5.5ft/hr. There are no violations at this time.
1397068474 2014-04-14 Casing String Witnessed No Violations I was on location to witness the installation of the tapered production casing. During my inspection the rig was installing the 4.5 WTC-80 casing. They are going to be installing 15 open hole packers and frac ports in the 4.5 casing. They will then taper the casing up to 5.5 ET-80 for the vertical section of the well. Aluminum rigid centralizers are being run on the 4.5 casing and bowstring centralizers on the 5.5 casing. Installing the casing will continue into tomorrow as total depth is 7542.
1397680984 2014-04-15 Drill / Deepen / Reopen No Violations During my inspection I found the Highlands drilling rig to be laying down the 5.5 production casing. The 4.5 casing got tight in the hole 400 into the lateral. They tried to wash down the casing but were only able to pressure up to 1200 psi due to the casing anchor in the production string. They will continue to lay down the casing and run back in the hole with a trichone bit to perform a cleanup cycle.
1398109914 2014-04-21 Drill / Deepen / Reopen No Violations During my inspection I found the Highlands rig to be stacked on trucks to be moved off location. The rig was on location ready to be trucked off along with the mud pumps and compressors. The pits were currently be cleaned and transferred to the storage pit using a flocculation aid. Chipco service rig will be brought on location Thursday to set up for the cementing of the production casing next week. The pits will be solidified when the water and mud has settled out clearing the water for disposal in an injection well. There are no violations at this time.
1399038965 2014-04-28 Casing String Witnessed No Violations I was on location to witness the cementing of the 4.5/5.5 tapered production string. This casing is using a open hole packer system with a cement port in the 5.5 vertical section at 4650. 192sks of Class A 2% CaCl and Gel were pumped into the well through 2.375 tubing. Circulation was established with 20 bbls of water. The cement was then pumped, and displaced with 16 bbls of fresh water. The well was then reversed circulated to clear the tubing and ensure the cement is on the backside of the 5.5 casing. They are expecting at least 1000 of cement coverage. Appalachian Well Services will be coming onto location tomorrow to run a cement bond log to determine cement top.

1400002584 2014-05-12 Completion Testing No Violations I was on location to follow-up on the hydraulic fracturing of this well. The frac crews are completed and are off location. The Chipco service rig is on location currently drilling out the frac ports to prepare the well for production. The blending truck spill that occurred during the stimulation has been completely cleaned up and the contaminated soil has been dug up and removed off location to a landfill. There are no violations at this time.
1401990140 2014-06-05 Production Wells No Violations I was on location to check the status of putting this well into production. The pumpjack has been installed and set to have a 10 stroke during production. The wellhead and tree valve assembly is currently being installed. The flow line for the well is on location and awaiting to be dug below grade. They expect the well to start producing next week.
1402905128 2014-03-26 Drill / Deepen / Reopen No Violations During my inspection I found the rig is currently down and not drilling due to the auto drill malfunctioning. The rig is currently on bottom circulating at 5935. They plan to drill ahead when the auto drill is fixed.
1437569415 2014-04-16 Drill / Deepen / Reopen No Violations During my inspection I found the Highlands drilling rig to be laying down the 4.5 production casing. The rig had to stop pulling casing at 6am due to pump on the rig and a generator failing. Some new parts are on route to the rig and when the rig issues are corrected crew will continue to lay down casing. They have received a load of 4 drill pipe to run into the hole with a trichone bit and a roller reamer to clean out the hole and allow the running of the production casing without hitting any of the tight spots.
1478314565 2014-12-11 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The lease road is graveled and leads up to a pumpjack well with ID sign and proper gauges and mechanical pop off. The tank battery is surrounded by a dike and contains ID sign, two 210bbl production tanks, two 210bbl brine tanks and a vertical separator. The location has been seeded and vegetation has been established. At this time preliminary restoration has been passed.
1544269491 2014-06-17 Production Wells No Violations I was on location to check the work being done on this well bi Chipco LLC service rig. The rig was on location to pull the rods and replace the downhole pump for this well. They will replace the pump and put the well back into production when the work is completed. There are no violations at this time.
2043299447 2014-02-25 Drill / Deepen / Reopen No Violations I inspected this drill site as a follow up to the diesel release and clean up on 4/24/2014. I found that the affected area had been satisfactorily cleaned up. The defective valve had been replaced, all soil was removed and replaced with new topsoil and slag rock. The Highlands Drilling Rig #7 was drilling at 458 KB. I did not identify any violations during this inspection.
2130897912 2014-05-21 Completion Testing No Violations I was on location to check the status of flow back for this well. Currently the well is flowing back at 25 barrels per hour. There is a three phase separator on location so that the gas flow is not vented into the atmosphere. The timetable for flow back on this well is 2 weeks but it is still not clear how long it will take place for.
