Breakdown was achieved at 2100psi and the average treating pressure was 3534psi at 18.7 barrels per minute (bpm) with a maximum of 21.9bpm. A volume of 1202bbls were pumped in the job, 833 barrels of which were for treatment. The instant shut in pressure was 3182psi, 5 minute 3097psi and 10 min shut in pressure 3028psi. The well was immediately flowed back when the Universal frac equipment was removed from location.

0296126409 2019-05-02 Preliminary Restoration No Violations My inspection found the pits have been filled and the land restored around the well and tank battery locations. There are no signs of active erosion at either location. The preliminary restoration is compliant with Ohio Revised Code 1509.072(A). The preliminary restoration is passed. The well is equipped with a pumpjack. The tank battery consists of three 210-barrel tanks, two horizontal separators and a plastic brine tank. The well is legibly identified. No compliance issues are cited at this time.

0739582400 2014-09-03 Drill / Deepen / Reopen No Violations I was on location to check the status of Advent drilling out their plug back cement plug. The rig was currently tripping back into the hole and blowing the hole down every 4 stands to displace out the brine in the hole from setting the plug. They will drill through the Clinton into the Queenston before tripping out and running back into the hole with directional tools and drilling ahead. There are no violations at this time.
0889382430 2014-09-26 Completion Testing No Violations I was on location to witness the hydraulic fracturing of this well. Just as formation break was achieved during pumping by Universal Well Services the pumps pressured up and could not pump anymore fluid. It was determined that the gel and sand used in the frac has plugged and blocked the perforations from taking anymore fluid. A rig will be used to run tubing into the well, circulate the gel out and repot acid on the perforations to clean up the wellbore. There are no violations at this time.
0975139655 2015-04-21 Preliminary Restoration No Violations I was on location to check the status of preliminary restoration. All equipment besides what is used for the production of the well has been removed from the location. The topsoil has been redistributed on the location and silt fence installed on the edges of the location. The gravel on location has been removed with exception for the lease road to the pumpjack. The location has not had vegetation established on it at this time. When vegetation has been established or a crop planted preliminary restoration shall be passed.
1126092854 2014-08-19 Administrative Inspection No Violations Mike Burkhart of Enervest contacted me to inform me that Advent drilling will be moving onto location 8/22/14 to rig up the well and begin drilling. They expect to spud the well 8/23/14.
1239526296 2014-08-25 Casing String Witnessed No Violations I was on location to witness the cementing of the 23 lb/ft 8.625 diameter intermediate casing. Advent drilled an 11 diameter hole to 1050 and installed the casing to 1013 with 7 centralizers and a guide shoe. Universal pumped 370sks of Class A 3% CaCl cement and displaced it with 64bbls of freshwater. There were 15bbls of good cement returned to surface and the cement held level. The rig will wait on cement for 8 hours before testing their blow out preventer. There are no violations at this time.
1401653510 2014-08-25 Drill / Deepen / Reopen No Violations I was on location to check the status of Advent drilling their 11 diameter hole. The rig is currently drilling on fresh water mud at 889. Their 8.625 diameter casing point will be 1000. They expect to reach this depth and begin running casing at 2pm today. Universal Well Services will cement the well. The newly located pits are in good condition and do not appear to have any water infiltration. There are no violations at this time.
1411480395 2014-09-23 Preliminary Restoration No Violations I was on location to witness the closing of the blow pit for this well. The pit has been solidified to a firm state and has no free water in it. A Universal crew was on location with a Chipco rig to spot 1000 gallons of 15% acid through 2 3/8 diameter tubing. The acid was displaced with 19bbls of brine and the well will be perforated later this week and fracked on 9/26/14. There are no violations at this time.
1646527123 2014-08-26 Drill / Deepen / Reopen No Violations I was on location to check the status of Advent drilling the vertical section of this directional well. The rig is currently drilling a 7.865 diameter hole at 1250 while drilling on air. The working pit has pumped empty and relined before drilling started. The blow out preventer was tested and passed. There are no violations at this time.
2007442843 2014-09-01 Administrative Inspection No Violations I was contacted by Joe Wade of Enervest explaining that their have been issues with their directional drilling tools and have drilled several large doglegs in the well. They have decided that they must plug back the well and sidetrack it. They pumped the cement plugs through 4.5 OD open ended drill pipe. A 400 plug will be set covering the Queenston and the Clinton as required, a brine spacer will be set and an additional 250 kick off plug will be set. They will drill into the cement with a trichone to dress the cement and then will run in with directional tools and kickoff the plug.
2053639728 2014-09-17 Casing String Witnessed No Violations I was on location to witness the cementing of the 4.5 diameter P-110 production casing for this well. A Chipco service rig ran 6520 of 2.375 diameter tubing into the hole and latched into the cement port collar and pumped 474sks of Unifill Light 3% CaCl, 6% gel and displaced the cement with 24bbls of fresh water. The cement port collar was shut and the tubing removed from the well. This cement job will cover the Point Pleasant formation up and 1000 over the Clinton Sandstone. There are no violations at this time.
2074244238 2014-08-21 Surface Facility Construction No Violations I was contacted by Mike Burkhart of Enervest (304) 989-7294, regarding the digging of the pits for this well. The planned location of the pits had a very large amount of porous sand and started having a large quantity of water infiltration. The sandy soil was very soft and acted like quicksand when trying to dig and work with it. It was decided that the pits be moved to the south side of the location. A test hole was dug and good clay soils were found with no water infiltration. They are going to dig a 80x100 pit and dig it shallower then normal to avoid hitting the water table. A berm will also be built up around the pits, although as required by law, the pit will not be allowed to fill above ground level. There is a possibility that field tile be encountered while digging the pits. The field tile will be capped and diverted around the pits so that the water from the field does not flow towards and into the pits. They will continue to dig the pits and prepare the location today and tomorrow.
