1185348681 2014-05-13 Drill / Deepen / Reopen No Violations I was on location with Inspector Roush to check the status of drilling. The location has been flooded out due to the heavy rains that came through the area. The stream near location swelled and flooded the location. Poulson Drilling has dug a relief pit to help empty the drilling pits that have been full with water due to the flooding. They are going to have to empty and reline the pits because a large amount of water has flowed below the pit liners. They expect to begin drilling again later this night.
1400524063 2014-05-19 Casing String Witnessed No Violations I was on location to observe the placement and cementing of the conductor casing. Poulson Drilling drilled a 20 hole to 185 , then ran 16 casing to 161. Universal Well Services cemented with 245 sacks of Type 1 cement blended with 3% CaCl2, 0.25 Lbs. / sack celloflake. I observed 18 barrels of cement returned to the surface.No violations were identified during this inspection.
1721895842 2015-09-16 Final Restoration No Violations I was on location to pass final restoration for this lost hole. This well was a lost hole and the drilling rig was skidded to permit #34151258660000 and the well is currently producing and has had preliminary restoration passed. This well has been properly plugged and at this time final restoration has been passed.
