0161292385 2015-05-28 Preliminary Restoration No Violations I was on location to check the status of preliminary restoration. The lease road is stable and leads up to a fenced wellhead with required I.D. signage. The well has a pumpjack on it but is currently running as a plunger lift. The tank battery contains two 140bbl tanks, poly brine tank, separator and is contained within a fence and a dike. An I.D. sign is visible on the fence of the tank battery. The sales line for the well has been installed, and the right-of-way seeded. The area around the wellhead has been tilled by the farmer for planting. When vegetation has been established preliminary restoration will be passed.
0322200966 2014-08-13 Casing String Witnessed No Violations I was on location to witness the cementing of the 11.75 diameter 42 lb/ft surface casing. Wildcat drilling drilled a 14.75 diameter hole to 520 and installed 510 of 11.75 diameter casing with 5 centralizers. Petroset pumped 365sks of Class A 3% CaCl with .5lb of cell-o-flake. The cement was displaced with 59bbls of fresh water and 28bbls of cement were returned to surface. There are no violations at this time.
1226031985 2014-08-14 Drill / Deepen / Reopen No Violations I was on location to witness the testing of the Blow Out Preventer (BOP). The BOP was pressured up to 450psi and held for 30 minutes. At the end of the 30 min test the 8.625 casing and BOP was holding at 410psi. This is within the allowable 10% decline and the BOP test has been passed. There are no violations at this time.
1408378108 2014-08-18 Casing String Witnessed No Violations I was on location to witness the cementing of the 10.5 lb/ft 4.5 production casing. The Wildcat rig drilled a 7.865 hole to 5090. They then installed the casing with 6 centralizers to 5050. Petroset began circulating the well when the cement head formed a leak and failed. Another cement head was brought onto location and 250sks of Petropic 10-4-2 cement was pumped and displaced with 80bbls of fresh water. The plug was landed and pressured up to 1500psi and held for 5 min. The floats held and flowed back .5bbl. The rig will now be moving to Coshocton County.
1410355934 2014-09-09 Completion Testing No Violations I was on location to witness the hydraulic fracturing of this well by Producers Service Corp. A total of 129916 gallons of water and additives were pumped into the well. Break was achieved at 1171psi and the average treating psi was 1719 at an average rate of 39 barrels per minute. The instant shut in pressure was 1441psi and the 5 minute shut in pressure 1171psi. There are no violations at this time.
1422893067 2015-02-02 Preliminary Restoration No Violations I was on location to check the status of preliminary restoration. The lease is stable and leads up to a fenced wellhead with required I.D. signage. The well has a pumpjack on it but is currently running as a plunger lift. I will follow up on location when there is no snow cover to ensure the location is seeded and will pass preliminary restoration then.
1434383776 2015-06-15 UrbanProduction Wells No Violations I was on location to follow up to complaint #5048. I received a phone message from Gavin Armstrong of the Ohio EPA stating that an odor complaint has been received near this well in Louisville, OH. The complainant states that gas coming from the Sylvester well owned by Ohio Valley Energy is making the neighborhood smell of oil odor. I talked to Don Neffin of OVE and he stated that the Ergon tanker driver left the latch open to the oil tank and the dump valve on separator was stuck open. The problems have been corrected and the complainant can no longer smell the oil and gas operations. There will be an additional charcoal filter installed on the tank battery to prevent odor from escaping. The tank battery is currently no longer leaking gas and the complaint can be closed.

1706783357 2014-08-12 Casing String Witnessed No Violations I was on location to witness the cementing of the 16 OD conductor casing. The Wildcat Rig drilled a 22 hole to 48 and installed 39 of 65 lb/ft casing. Petroset pumped 200sks of Class A cement and displaced it with 8bbls of freshwater. 9bbls of cement were returned to surface. The rig will wait on cement 8 hours and then drill ahead to at least 50 below the base of the Big Injun.
1810131153 2014-08-25 Completion Testing No Violations I was on location to check the status of this drilling location. Currently a bulldozer is preparing the location for the frac crews to set up and stimulate the well. The pits are open but have had a large quantity of fluid pulled off of them. Erosion controls are established and working following spells of heavy rain. There are no violations at this time.
