0499203045 2017-08-17 Storage Well No Violations At the time of this inspection, the well site is equipped with a 4.5 i.nch casing and a swage with a bull plug and topped with a chart reader. This well is used as a monitoring well and does not have an associated tank. Signage is needed at the well location. The lease access is stable and drivable. I contacted Dominion East Ohio about installing signage at the well location.
1481206180 2016-12-07 Storage Well No Violations I conducted a final restoration inspection on this well. The well has not been plugged and abandoned (PA). The well has valve and a swage with a meter attached. I checked the plug record and it states the well was reopened, drilled out and to be completed as a Berea Sandstone gas monitoring well. I e-mailed Steve Opritza (ODNR Oil & Gas) with the information I found. He returned an e-mail stating he had changed the status from PA to storage well after speaking with Brian Morley of Dominion East Ohio Gas. The RBDMS coordinates were used to locate the well.
2040500388 2018-06-06 Storage Well No Violations I did a status check inspection for this well. The well is being used as a monitoring well. There is no tank battery. The well is legibly identified.
