| 0803378810 | 2014-06-17 | Orphan | No Violations | This follow up inspection was conducted on 06-17-14 to compile information for an orphan case file. Complaint #4699 stated this historical oil and gas well has impacted a water well at address 12436 Edison Road with natural gas. This case file was initiated by Kyle Tharp, Hydrogeologist for ODNR, in May of 2014. On this inspection, I met with Mr. Pauli and we viewed both the water well and the oil and gas well in question. I then explained the Orphan Well Program and our plugging schedule to Mr. Pauli. I also obtained the following information:
GPS coordinates for: 1. Rig access through gate : 40.94524, -81.16044
2. Affected water well : 40.94780, -81.16060
3. Orphan well: 40.943775, -81.161750
-81.16044. Access for plugging operation will be through a pasture which will not be used during plugging operation. This pasture area is usually wet and it will probably be necessary to construct a temporary road or install terra-mats to access the location.
There is no other equipment associated with this well present on-site. The driveway for addresses 12434/12436 has two cattle grates to prevent cattle from crossing. The following distances were aquired:

| 0976276882 | 2014-08-07 | Orphan | No Violations | This follow up inspection was conducted with Mr. Orville and Nick of Nicks Well Plugging on 08-07-14 to view the well site and review the plugging specifications so Nick can submit his bid for plugging this well. I showed Nick the access route to the well, which is south off of Edison Road (State Route 619) through the farm gate and into the active pasture for the cattle. The well is situated in the flood plain of Little Beach Creek. This area is usually wet most of the year. Plugging operation should be done when dry or frozen. If done when wet, terra mats may be required. A well head control device will be required during plugging operation. There is an obstruction at a depth of 98 which will need to be drilled out to reach total depth or a depth approved by Division. The contractor will be required to seed with mixture that is appropriate for this pasture. |
| 1134620336 | 2014-05-08 | Production Wells | No Violations | I was on location to follow up to complaint #4699. Complainant states there is gas in his water well. He can hear the bubbling in the well. He also said you can light his water at the faucet on fire. This well was drilled in 1919 to a depth of 4778 in the Clinton formation and was abandoned in 1922. He said the well has never been plugged and there is a 10 diameter casing showing at the ground level. During my inspection I found 10 casing present at this location. The well card states 10 casing was set to 115.5 and 8 casing to 731.2. The 8 casing was not visible from surface. The well is full of water and has bubbling methane coming out of it. My PHD6 gas detector was used to confirm that the bubbling gas is indeed methane. The casing is at ground level. There is no production equipment associated with this well. The well is located next to a stream running through a cattle pasture. There is no oil present around the well or in the nearby stream. An additional water well investigation will be conducted by Kyle Tharp and this well has been referred to the Orphan Well Program. |

The landowner contacted Division of Oil & Gas Hydrologist Kyle Tharp and informed him his water has gas in it. I met the landowner on location and we went to the plugged wells location. There was no visual ground evidence of plug failure. The well site has green vegetation on it. I advised the landowner any further investigation which may be needed would be conducted by the Hydrologist. This well is associated with Complaint # 4699 and the well was plugged the Division of Oil & Gas -Orphan Well Progran. Inspector Richard Kepler witnessed the plugging on 02/29/2016. |
