0906674737	2016-03-15	UrbanProduction Wells	No Violations	I inspected this the Van Devere # 1 well to access the corrective measures that were detailed in the previously issued Notice of Violation # 1407944798. The I. D. signage still was illegible and needs to be corrected. The 4.5 production casing still had a pipeline clamp on the top joint. The pipe clamp was not sufficient to prevent the escape of oil and gas. The top joint of 4.5 casing needs to replaced. The top joint of 1.5 tubing was sheared off during a flood event and welded back together. The top joint of 1.5 tubing will require replacement. The tank battery consisted of a 50 barrel oil, horizontal separator and a domestic gas connection with meter. The I. D. signage was illegible. The fencing around the tank battery was a lease negotiated item and was not required. None of the violations detailed in the previous notice ( # 1407944798 ) have been corrected. In order to bring the Van Devere # 1 well into compliance the Operator shall, 1.) post the required legible I.D. signage ( OAC 1501: 9-05 E-1 ), 2.) replace the top joint of 4.5 production casing and the top joint of 1.5 tubing ( ORC 1509: 12A and ORC 1509: 20 ). Failure to complete compliance requirements will result in further enforcement activities. Please call Bob Roush, within 30 days, at 330.690.8837 for a compliance inspection once the repairs are completed.

1407944798	2014-08-12	UrbanProduction Wells	Violations Noted	I inspected this well and associated equipment on 8/12/2014. The tank battery consists of a 50 barrel tank to contain all produced fluids and a 2408 horizontal separator with a orifice gas meter. This tank battery was recently rebuilt after a flood washed out and moved the tank. At the wellhead the 1.5 tubing was sheared off by the debris being carried down stream at the time. The wellhead is located 41 from the creek bed of Yellow Creek. During my inspection I identified numerous violation that shall require immediate resolution. First, the top joint of the 4.5 casing is leaking and has a temporary pipeline clamp on it. Secondly, the top joint of 1.5 was sheared off by a flood. A repair was made by welding the tubing back together. Thirdly, the required identification signage is illegible. Fourthly, the fencing surrounding the tank battery was removed by the flood waters. Remedial actions shall include 1.) Removal and replace of the compromised 4.5 casing, 2.) Removal and replacement of the top joint of 1.5 tubing, 3.) Re - establishing the fencing surrounding the tank battery, 4.) Posting the required identification signage at the tank battery. The prescribed remedial measures shall be completed within 60 days of receipt of this notice.
1462378751	2016-05-03	UrbanProduction Wells	No Violations	I inspected the Van Devere # 1 well for a Notice of Violation compliance status and found that the previously prescribed corrective measures were not performed. The Notice of Violation ( # 1407944798 ) specified measures to be performed to bring the well into compliance with 1509 ORC. Those measures included 1) place legible I.D. signage at the tank battery, 2) replace the fencing around the tank battery, 3) well is insufficiently equipped to prevent the escape of oil and gas, 4) defective casing, leaking well. The odor of natural gas is noticeable around the wellhead. Without immediate corrective measures being performed further enforcement activities will be pursued.
